摘要
目的:观察膝关节痛风性关节炎患者应用药物保守治疗联合关节镜清理术治疗的效果。方法:选取2020年4月—2022年4月贵州安顺七十三医院收治的膝关节痛风性关节炎患者52例为研究对象,按照治疗方式分为两组,各26例。对照组给予药物保守治疗,试验组在对照组基础上给予关节镜清理术治疗。比较两组治疗前后相关炎性因子、红细胞沉降率、血尿酸、治疗效果以及不良反应情况。结果:治疗后,两组患者C反应蛋白、白细胞介素-6和白细胞介素-8水平均降低,试验组水平低于对照组,差异具有统计学意义(P=0.001);治疗后,两组患者红细胞沉降率、血尿酸水平均降低,试验组低于对照组,差异具有统计学意义(P=0.001);试验组患者治疗效果总有效率高于对照组,差异具有统计学意义(P=0.013);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:膝关节痛风性关节炎患者在保守治疗基础上联合关节镜清理术治疗效果优于药物保守治疗,可明显改善炎性因子水平,且安全性高。
Objective:To observe the effect of conservative drug therapy combined with arthroscopic debridement in patients with gouty arthritis of knee joint.Methods:Fifty-two patients with gouty arthritis of knee joint admitted to Anshun Seventy-third Hospital from April 2020 to April 2022 were selected as the study objects,and were divided into two groups according to treatment methods,with twenty-six cases each.The control group was given conservative drug therapy,and the experimental group was given arthroscopic debridement based on the control group.The relevant inflammatory factors,erythrocyte sedimentation rate,blood uric acid,therapeutic effect and adverse reactions were compared between the two groups before and after treatment.Results:After treatment,the levels of C-reactive protein,interleukin-6 and interleukin-8 were decreased in both groups,and the levels in the experimental group were lower than those in the control group,the difference was statistically significant(P=0.001).After treatment,erythrocyte sedimentation rate and blood uric acid level were decreased in both groups,and the experimental group was lower than the control group,the difference was statistically significant(P=0.001).The total effective rate of treatment in experimental group was higher than that in control group,the difference was statistically significant(P=0.013).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The effect of joint arthroscopic debridement on gouty arthritis of knee joint based on conservative therapy is better than that of drug conservative treatment,which can significantly improve the level of inflammatory factors and has high safety.
