摘要
糖尿病黄斑水肿(DME)归属于中医“水肿”“视瞻昏渺”“视直如曲”的范畴,结合眼底微观辨证认为,阳气郁滞贯穿DME始终,水湿内停、营卫失调、瘀血阻滞为病机关键,病证后期痰瘀互结黄斑,成顽症痼疾、昭示预后。基于通阳法源流考、DME中医机理分析,治疗当以通阳解郁、平衡阴阳为原则,行气解郁通阳、祛水除湿通阳、调和营卫通阳、活血化瘀通阳、化痰散结通阳为关键,泄热除闭通阳为辅助,同时始终不忘通阳守阴、升阳明目,使阳通郁解、瘀化湿除、营卫和调,达到帮助黄斑水肿消退、促进出血渗出吸收、提高视功能的目的。
Diabetic macular edema(DME) belongs to the category of "edema" "blurred vision" and " seeing straight as crooked" of Traditional Chinese Medicine(TCM).Combined with the microscopic syndrome differentiation of fundus,it is believed that Yang Qi stagnation is throughout DME,water and dampness retention,disorder of nutrient and defense,and stagnation of blood stasis are the key pathogenesis.In the late stage of the disease and syndrome,phlegmblood stasis binding in macula,which makes the disease stubborn and obstinate with predictable poor prognosis.Based on the origin research of unblocking Yang method and the analysis of TCM mechanism of DME,the treatment of DME should be based on the principle of unblocking Yang to alleviate depression and balancing Yin and Yang,the key of treatment is promoting the circulation of Qi and releasing depression to unblock Yang,removing water and dampness to unblock Yang,regulating nutrient and defense to unblock Yang,promoting blood circulation and remove blood stasis to unblock Yang,dispelling phlegm and dissipating binds to unblock Yang,and purging heat and removing obstruction to unblock Yang as the adjuvant therapy.At the same time,we should never forget to unblock Yang to guard Yin and raise Yang to brighten the eyes.And thereby,Yang unblocked and depression released,stasis resolved and dampness relieved,nutrient and defense in harmony which lead to subside of macular edema,promoted bleeding exudation and absorption,and improved visual function.
