血清尿酸、同型半胱氨酸预测急性缺血性脑卒中患者静脉溶栓后出血转化的价值 被引量:1

Value of Serum Uric Acid and Homocysteine in Predicting the Transformation of Hemorrhage after Intravenous Thrombolysis in Patients with Acute Ischemic Stroke

摘要 目的探讨血清尿酸(UA)、同型半胱氨酸(Hcy)预测急性缺血性脑卒中(AIS)患者静脉溶栓后出血转化(HT)的价值。方法选取2021年12月至2022年12月我院收治的100例AIS患者,根据静脉溶栓24h后HT情况分为出血组和未出血组。检测两组血清UA及Hcy水平,分析其与AIS静脉溶栓后HT的关系。结果出血组UA水平低于未出血组,Hcy水平高于未出血组(P<0.05)。Logistic多因素回归分析显示Hcy为影响静脉溶栓后HT的独立危险因素,UA为HT的保护因素(P<0.05)。UA、Hcy及联合检测诊断静脉溶栓后HT的AUC分别为0.770、0.887、0.956,联合检测诊断的AUC高于UA、Hcy单独检测(P<0.05)。结论血清UA、Hcy与AIS患者静脉溶栓后HT关系密切,溶栓前检测二者水平有助于早期预测溶栓后HT。 Objective To explore the value of serum uric acid(UA)and homocysteine(Hcy)in predicting hemorrhagic transformation(HT)after intravenous thrombolysis in patients with acute ischemic stroke(AIS).Methods 100 AIS patients admitted to our hospital from December 2021 to December 2022 were divided into bleeding group and non-bleeding group according to the HT level at 24 hours after intravenous thrombolysis.The serum UA and Hcy levels of both groups were detected,and the correlation with HT after AIS intravenous thrombolysis was analyzed.Results The UA level of the bleeding group was lower than that of the non-bleeding group,and the Hcy level was higher than that of the non-bleeding group(P<0.05).Logistic multivariate regression analysis showed that Hcy was independent risk factor affecting HT after intravenous thrombolysis,and UA was the protective factor of HT(P<0.05).The AUC of UA,Hcy and combined detection for diagnosis of HT after intravenous thrombolysis were 0.770,0.887 and 0.956,respectively.The AUC of combined detection for diagnosis was higher than that of UA and Hcy alone(P<0.05).Conclusions Serum UA and Hcy are closely related to HT after intravenous thrombolysis in AIS patients,and the detection before thrombolysis is helpful for early prediction of HT after thrombolysis.
