正常人腕掌部尺神经多模态超声评估

Multimodal ultrasound assessment of the ulnar nerve at the palmar part of the wrist in normal individuals

摘要 目的获得腕掌部尺神经主干、深支、浅支及其周围小鱼际肌的的超声影像及小鱼际肌的杨氏模量值,为研究腕掌部尺神经病变奠定影像诊断基础。方法选取30名健康志愿者(男18名,女12名),共60支手腕。测量并记录受试者双侧手腕宽度、手腕深度、手腕周长、手掌长度及手掌宽度;行超高频超声检查,测量并记录腕尺管内尺神经主干及其深支、浅支的横截面积(CSA);利用微细血流成像技术观察尺神经及其周围的血流信号分布情况,并应用剪切波弹性成像技术分别获取小鱼际肌的杨氏模量值。结果健康成年男性与女性间手腕宽度,手腕深度,手腕周长,手掌长度及手掌宽度与尺神经主干及其深、浅支的横截面积间的差异有统计学意义(P<0.05);尺神经主干及其深、浅支的横截面积与手腕宽度,手腕深度,手腕周长,手掌长度及手掌宽度相关(P<0.05);优势手与非优势手在尺神经主干及其深、浅支横截面积上无明显统计学差异(P>0.05);所测全部尺神经及其深、浅支微细血流显像均未探及血流信号;除掌短肌外的小鱼际肌的杨氏模量值在男女性别间、优势手及非优势手间无明显统计学差异(P>0.05)。结论尺神经主干及其深、浅支的横截面积在不同的性别间存在差异,且与手腕的粗细、手的长度、手的宽度等有关。而优势手与非优势手在尺神经主干及其深、浅支横截面积上无明显统计学差异。腕掌部尺神经病变超声检查时,对同一人的双侧腕掌部尺神经各项参量及小鱼际肌进行对比扫查更有意义。 Objective To obtain ultrasound images of the main trunk,deep branch,superficial branch of the ulnar nerve at the palmar part of the wrist and the hypothenar muscle around it,as well as the Young's modulus values of the hypothenar muscle in order to lay the foundation for imaging diagnosis of ulnar nerve lesion at the palmar part of the wrist.Methods 30 healthy volunteers(18 males and 12 females)were selected,with a total of 60 wrists.The bilateral wrist width,wrist depth,wrist circumference,palm length and palm width of the subjects were measured and recorded.Super high frequency ultrasound was performed to measure and record the cross-sectional area(CSA)of the main ulnar nerve trunk and its deep and superficial branches in the ulnar canal of wrist.The distribution of blood flow signals in and around the ulnar nerve was observed by using micro blood flow imaging technology.And the Young's modulus values of the hypothenar muscle were obtained by using shear wave elastic imaging technology.Results There were statistically significant differences in wrist width,wrist depth,wrist circumference,palm length and palm width,and the cross-sectional area of the ulnar nerve trunk and its deep and superficial branches between healthy adult males and females(P<0.05).The cross-sectional area of the ulnar nerve trunk and its deep and superficial branches was correlated with wrist width,wrist depth,wrist circumference,palm length and palm width(P<0.05);There was no statistically significant differences in the cross-sectional area of the ulnar nerve trunk and its deep and superficial branches between the dominant and non-dominant hands(P>0.05).All measured ulnar nerves and their deep and superficial branches showed no blood flow signals on the micro blood flow imaging.There was no statistically significant difference in the Young's modulus values of the hypothenar muscle,except for the palmaris brevis,between males and females,and between the dominant and non-dominant hands(P>0.05).Conclusion The cross-sectional area of the main
