摘要
目的:观察卧位偏向牵引联合中药熏蒸治疗神经根型颈椎病的临床疗效。方法:将80例神经根型颈椎病住院患者随机分为2组,每组40例。2组患者均采用软伤外洗1号方(珍珠透骨草、伸筋草、威灵仙、千年健、香加皮、海桐皮、醋三棱、醋莪术、白芷、炒桃仁、红花、苏木、花椒、艾叶)进行颈部中药熏蒸治疗,对照组联合坐位传统枕颌带牵引治疗,治疗组联合卧位偏向牵引治疗,2次/d,共治疗28次。2组患者均连续治疗2周,收集并统计分析治疗前后颈椎病症状量表、颈椎功能障碍指数(Neck Disability Index,NDI)、视觉模拟量表(Visual analogue sacle,VAS)、SF-36生活质量评分、斜方肌弹性模量的变化情况。结果:治疗后,2组患者颈椎病症状评分明显升高,VAS评分明显下降,ODI指数均下降,SF-36量表评分(除精神健康项目以外,生理机能、生理职能、躯体疼痛、总体健康、活力、社会功能及情感职能以上7个子项目)升高明显,组间比较差异具有统计学意义。结论:卧位偏向牵引联合颈部中药熏洗治疗神经根型颈椎病,可以有效地缓解颈肩部及上肢疼痛情况,缓解颈椎病临床症状,改善颈椎及上肢功能障碍,并改善患者生活质量。
Objective:To observe the clinical curative effect of lateral position traction combined with traditional Chinese medicine fumigation in the treatment of cervical spondylotic radiculopathy.Methods:80 hospitalized patients with cervical spondylotic radiculopathy were randomly divided into control group and treatment group,40 cases in each group.The two groups of patients were all treated with soft injury Waixi No.1 prescription(Zhu Gucao,Shenjincao,Clematis,Qiannianjian,Xiangjiapi,Pittosporum,Vinegar Sanleng,Curcuma Curcuma,Angelica dahurica,Stir-fried peach kernel,Red Chinese herbal fumigation for the neck,Sapphire,Sapphire,Zanthoxylum bungeanum,and Artemisia argyi.The control group was combined with traditional occipital traction in sitting position,and the treatment group was combined with lateral traction in lying position,twice a day,for a total of 28 sessions.The two groups of patients were treated continuously for 2 weeks,and the cervical spondylosis symptom scale,cervical dysfunction index(Neck Disability Index,NDI),visual analogue scale(Visual analog sacle,VAS),SF-36 quality of life before and after treatment were collected and analyzed statistically.Changes in score and elastic modulus of trapezius muscle.Results:After treatment,the symptom score of cervical spondylosis in the two groups was significantly increased,the VAS score was significantly decreased,and the ODI index was decreased.The above 7 sub-items,vitality,social function and emotional function) increased significantly,and the difference between groups was statistically significant.Conclusion:The treatment of cervical spondylotic radiculopathy with lateral traction in supine position combined with neck traditional Chinese medicine fumigation can effectively relieve neck,shoulder and upper limb pain,relieve clinical symptoms of cervical spondylosis,improve cervical spine and upper limb dysfunction,and improve the quality of life of patients.
