摘要
目的:研究探讨解毒清肠汤联合西药治疗慢性结肠炎的临床疗效及对血清细胞因子的影响。方法:选取2019年1月~2021年1月重庆市涪陵中心医院收治的120例慢性结肠炎患者随机数字表法分为观察组、对照组,各60例。对照组采用美沙拉嗪为主的西药治疗,观察组在对照组基础上加用解毒清肠汤治疗。评估比较两组患者的临床疗效、中医证候积分、血清细胞因子、T淋巴细胞亚群水平、不良反应发生率。结果:观察组总有效率为91.67%(55/60),高于对照组患者的76.67%(46/60),差异有统计学意义(P<0.05);治疗后观察组患者的中医证候包括腹痛腹泻、腹胀食少、腰膝酸软、神疲乏力等积分均低于对照组(P<0.05)。治疗后观察组患者的中性粒细胞明胶酶相关脂质运载蛋白(NGAL)、C反应蛋白(CRP)水平均低于对照组,肠黏蛋白2(MUC2)高于对照组(P<0.05);治疗后观察组患者的T淋巴细胞亚群CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于对照组,而CD8^(+)则低于对照组(P<0.05);两组患者治疗期内不良反应发生率无明显差异性(P>0.05)。结论:解毒清肠汤联合西药治疗慢性结肠炎患者的疗效良好,能够改善患者的中医证候、血清细胞因子和T淋巴细胞亚群水平,安全性良好,值得临床推荐。
Objective:To study the clinical effect of Banxia Xiexin Decoction Combined with western medicine in the treatment of chronic colitis and its influence on serum cytokines.Methods:120patients with chronic colitis in our hospital were selected from January 2019to January 2021.According to the random number table method,they were divided into the control group and the observation group,with 60cases in each group.The clinical efficacy,TCM syndrome score,serum cytokines,T lymphocyte subsets and the incidence of adverse reactions of the two groups were evaluated and compared.Results:The total effective rate of the observation group was 91.67%(55/60),higher than 76.67%(46/60)of the control group(P<0.05).The scores of TCM symptoms including abdominal pain,diarrhea,abdominal distension,less food,soreness and weakness of the waist and knee(P<0.05).The levels of NGAL,MUC2and CRP in the observation group were lower than those in the control group(P<0.05).T lymphocyte subsets CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the observation group were higher than those in the control group,while CD8^(+)was lower(P<0.05).During the treatment,the incidence of adverse reactions in the observation group and the control group were 11.67%(7/60)and 8.33%(5/60),respectively(P>0.05).Conclusion:Banxia Xiexin Decoction Combined with western medicine has a good therapeutic effect on patients with chronic colitis,which can improve the TCM symptoms of patients,improve the level of T lymphocyte subsets of patients,and has good safety,which can be recommended clinically.
