摘要
自发性冠状动脉夹层是一种严重的、危及生命的急症心脏病,可无明显诱因发病,是心源性猝死的一个重要原因,以往被视为罕见疾病,且常因症状与急性冠脉综合征相似被误诊。如今其发病率日益上升,越来越多的研究表明自发性冠状动脉夹层是急性冠脉综合征的重要病因。既往认为发病人群主要为妊娠和围产期妇女以及患有肌纤维发育不良或某些结缔组织病的人群。自发性冠状动脉夹层的相关并发症主要包括心肌梗死、心源性猝死、室性心律失常以及其他与心肌缺血相关的梗死后机械性并发症。这些并发症严重影响患者预后。该文综述了目前关于自发性冠状动脉夹层的研究进展,主要包括流行病学、病理生理学临床特征、诊疗手段以及相关并发症等诸多方面,旨在为临床实践提供参考。
Spontaneous coronary artery dissection(SCAD)is a serious and life-threatening emergency heart disease,which may occur without obvious causes and is an important cause of sudden cardiac death.It was considered as a rare disease in the past and was often misdiagnosed because its symptoms were similar to acute coronary syndrome.With the increasing incidence,more and more studies have shown that SCAD is an important cause of acute coronary syndrome.In the past,it was thought that the main affected groups were pregnant and perinatal women and those with fibromuscular dysplasia or some connective tissue diseases.The related complications of SCAD mainly include myocardial infarction,sudden cardiac death,ventricular arrhythmia,and other post-infarction mechanical complications related to myocardial ischemia.These complications seriously affect the prog-nosis of patients.This article reviews the current research progress on SCAD,including epidemiology,pathophysiology,clinical characteristics,diagnosis and treatment methods,and related complications.
