摘要
目的:观察盐酸米诺环素软膏联合Vitapex糊剂治疗牙周-牙髓联合病变患者的效果。方法:选取2020年7月至2021年2月该院口腔科收治的70例牙周-牙髓联合病变患者进行前瞻性研究,以奇偶数法将其分为对照组和观察组各35例。对照组采用Vitapex糊剂治疗,观察组在对照组基础上联合盐酸米诺环素软膏治疗,比较两组临床疗效,治疗前、治疗8周后牙周相关指数[出血指数(BI)、探诊深度(PD)、菌斑指数(PLI)、附着水平(AL)]水平、龈沟液炎性因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]水平,以及疼痛程度[视觉模拟评分法(VAS)]评分。结果:观察组治疗总有效率为91.43%(32/35),高于对照组的71.43%(25/35),差异有统计学意义(P<0.05);治疗后,两组BI、PLI评分和PD、AL均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,观察组龈沟液IL-6、TNF-α水平均低于对照组,差异有统计学意义(P<0.05);治疗后,观察组VAS评分低于对照组,差异有统计学意义(P<0.05)。结论:盐酸米诺环素软膏联合Vitapex糊剂治疗牙周-牙髓联合病变患者可提高治疗总有效率,降低牙周相关指数水平、龈沟液炎性因子水平和VAS评分,效果优于单纯Vitapex糊剂治疗。
Objective:To observe effects of Minocycline hydrochloride ointment combined with Vitapex paste in treatment of patients with combined periodontal-endodontic lesions.Methods:A prospective study was conducted on 70 patients with combined periodontal-endodontic lesions admitted to the department of stomatology of the hospital from July 2020 to February 2021.They were divided into control group and experimental group by using the odd-even method,35 cases in each.The control group was treated with Vitapex paste,while the observation group was treated with Minocycline hydrochloride ointment on the basis of that of the control group.The clinical efficacy,the periodontal related index levels[bleeding index(BI),probing depth(PD),plaque index(PLI),attachment level(AL)],the gingival crevicular fluid inflammatory factor levels[interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)],and the pain degree[visual analogue scale(VAS)]were compared between the two groups before and 8 weeks after the treatment.Results:The total effective rate of treatment in the observation group was 91.43%(32/35),which was higher than 71.43%(25/35)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the BI and PLI scores as well as the PD and AL levels in the two groups were lower than those before the treatment,those in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of IL-6 and TNF-αin the gingival crevicular fluid in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,after the treatment,the VAS score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusions:Minocycline hydrochloride ointment combined with Vitapex paste in the treatment of the patients with combined periodontal-endodontic lesions can improve the total effective rate of treatme
