基于针刺脑效应的大脑默认网络研究

摘要 大脑默认网络(default mode network,DMN)与人的认知、情感密切相关,在多种疾病中均能发现其异常,而针刺脑效应研究也发现了DMN相关脑区的变化,DMN在针刺脑效应中发挥怎样的作用,是否是针刺机制的一种可能尚未明确。目前,人体生理和病理脑网络的机制仍在研究中,从分子生物学角度而言,DMN的细胞、神经结构并不清楚,大脑网络亦有其复杂性与交叉性,针刺脑机制研究的难度较大。未来可从神经元和分子水平上进行脑研究,将磁共振功能成像与电生理融合,从细胞水平验证其机制,研究大脑结构-功能连接及二者耦合关系。同时,扩大临床研究样本量,设计更加严谨、客观的试验,严格控制相关变量,规范操作,为后续针刺脑网络研究提供客观依据。 Brain′s default mode network(DMN)plays an important role in maintaining brain health and is closely related to human cognition and emotion.Studies on the effects of acupuncture on the brain also found different changes in the brain regions related to DMN,but the changes of DMN cannot explain the mechanism of acupuncture.The regulation of DMN by acupuncture can only reflect the indirect consequences of treatment,and the changes in DMN may be directly driven by the somatosensory effects of acupuncture,or indirectly caused by the emotional or cognitive processes associated with the therapeutic effect.The mechanism of human physiological and pathological brain network is still under study.From the perspective of molecular biology,the cellular and neural structure of DMN is not clear,and the brain network has its complexity and intersectionality,so it is difficult to study the mechanism of acupuncture in the brain.In the future,it is possible to conduct brain research at the neuronal and molecular levels,integrate functional magnetic resonance imaging with electrophysiology,verify its mechanism at the cellular level,study the structure-function connection of brain and their coupling relationship,and improve future acupuncture research.At the same time,the research sample size should be expanded,more rigorous and objective experiments should be designed,relevant variables should be strictly controlled,and the results should be interpreted in multiple dimensions by objectifying and digitizing the indicators,so as to further explore the biological and physiological mechanisms of the effects of acupuncture on the brain,so that acupuncture can play a more important role in clinical practice.
