摘要
目的探讨生物合成人胰岛素联合门冬胰岛素治疗妊娠期糖尿病(GDM)的临床效果。方法选取2018年9月至2019年9月我院妇产科100例GDM患者作为研究对象,按照随机数字表法将其分为对照组(55例)与研究组(45例)。对照组给予门冬胰岛素注射液治疗,研究组在对照组基础上联用生物合成人胰岛素治疗。比较两组的血糖水平、胰岛功能、炎症因子水平及妊娠结局。结果治疗后,研究组的糖化血红蛋白(HbA1c)、空腹血糖(FBG)及服糖后2 h血糖(FPG2)水平低于对照组(P<0.05)。治疗后,研究组的空腹C肽(FC-P)水平及胰岛β细胞功能指数(HOMA-β)高于对照组,空腹胰岛素(FINS)水平及胰岛素抵抗指数(HOMA-IR)低于对照组(P<0.05)。治疗后,研究组的超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)及脂蛋白相关性磷脂酶A2(Lp-PLA2)水平低于对照组,脂联素(APN)水平高于对照组(P<0.05)。研究组的不良妊娠结局总发生率低于对照组(P<0.05)。结论生物合成人胰岛素联合门冬胰岛素可有效改善GDM患者的血糖水平,提高胰岛β细胞功能,调节机体炎症因子水平,降低不良妊娠结局发生风险。
Objective To investigate the clinical effect of biosynthetic human insulin combined with insulin aspart in the treatment of gestational diabetes mellitus(GDM).Methods A total of 100 GDM patients in gynecology and obstetrics department of our hospital from September 2018 to September 2019 were selected as the research objects and divided into control group(55 cases)and study group(45 cases)according to the random number table method.The control group was treated with insulin aspart injection,and the study group was treated with biosynthetic human insulin on the basis of the control group.The blood glucose levels,islet function,inflammation factors levels and pregnancy outcomes were compared between the two groups.Results After treatment,the levels of glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG)and 2 h fasting plasma glucose after taking sugar(FPG2)in the study group were lower than those in the control group(P<0.05).After treatment,the fasting C-peptide(FC-P)level and pancreaticβ-cell function index(HOMA-β)in the study group were higher than those in the control group,and the fasting insulin(FINS)level and homeostasis model insulin resistance index(HOMA-IR)were lower than those in the control group(P<0.05).After treatment,the levels of hypersensitive-C reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and lipoprotein-associated phospholipase A2(Lp-PLA2)in the study group were lower than those in the control group,and the level of adiponectin(APN)was higher than that in the control group(P<0.05).The total incidence of odverse pregnancy outcomes of the study group was lower than that of the control group(P<0.05).Conclusion Biosynthetic human insulin combined with insulin aspart can effectively improve the blood glucose levels of GDM patients,improve the function of isletβcells,regulate the levels of inflammatory factors,and reduce the risk of adverse pregnancy outcomes.
