摘要
目的探讨重复经颅磁刺激联合呼吸功能训练在脑卒中后吞咽障碍中的应用效果。方法选取2019年1月至2020年12月本院收治的86例脑卒中后吞咽障碍患者作为研究对象,按照随机数字表法分为对照组与观察组,各43例。对照组给予常规吞咽康复治疗,观察组在对照组基础上给予重复经颅磁刺激联合呼吸功能训练治疗,比较两组治疗前后脑源性神经营养因子(BDNF)、胰岛素样生长因子(IGF-1)、神经轴突生长抑制因子-A(Nogo-A)、血管内皮生长因子(VEGF)水平及吞咽功能。结果治疗后,观察组BDNF、IGF-1及VEGF水平均高于对照组,Nogo-A水平低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组标准吞咽功能评定(SSA)评分低于对照组,差异有统计学意义(P<0.05)。结论重复经颅磁刺激结合呼吸功能训练治疗脑卒中后吞咽障碍效果显著,可有效改善患者神经功能及吞咽障碍,值得临床推广应用。
Objective To explore the application effect of repetitive transcranial magnetic stimulation combined with respiratory function training on dysphagia after stroke.Methods 86 patients with dysphagia after stroke admitted to our hospital from January 2019 to December 2020 were selected as the research subjects,and they were divided into control group and observation group according to random number table method,with 43 cases in each group.The control group was given routine swallowing rehabilitation treatment,and the observation group was given repetitive transcranial magnetic stimulation combined with respiratory function training on the basis of the control group.The levels of brain-derived neurotrophic factor(BDNF),insulin-like growth factor-1(IGF-1),neurite outgrowth inhibitor-A(Nogo-A),vascular endothelial growth factor(VEGF)and swallowing function were compared between the two groups before and after treatment.Results After treatment,the levels of BDNF,IGF-1 and VEGF in the observation group were higher than those in the control group,and the level of Nogo-A was lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the standardized swallowing assessment(SSA)score of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion Repetitive transcranial magnetic stimulation combined with respiratory function training has significant effect on the treatment of dysphagia after stroke,which can effectively improve the neurological function and dysphagia of patients,and is worthy of clinical application.
