摘要
目的分析小柴胡汤治疗系统性红斑狼疮(SLE)的临床疗效及对血清免疫指标与T细胞免疫球蛋白及黏蛋白分子水平的影响。方法选取2018年4月至2020年8月本院收治的88例SLE患者作为研究对象,采用随机数字表法分为参照组和观察组,每组44例。参照组给予利妥昔单抗治疗,观察组给予小柴胡汤治疗,比较两组临床疗效、不良反应发生率和治疗前后T细胞免疫球蛋白黏蛋白分子[T细胞免疫球蛋白黏蛋白域蛋白-1(TIM-1)、T细胞免疫球蛋白黏蛋白域蛋白-3(TIM-3)]、Toll样受体4(TLR4)蛋白水平、免疫指标[免疫球蛋白A(IgA)、免疫球蛋白M(IgM)、免疫球蛋白G(IgG)、补体C3、补体C4]水平。结果观察组治疗总有效率(95.45%)高于参照组(79.55%),差异有统计学意义(P<0.05)。观察组不良反应发生率(4.55%)低于参照组(20.45%),差异有统计学意义(P<0.05)。治疗后,观察组IgA、IgM、IgG、补体C3水平均低于参照组,补体C4水平高于参照组,差异有统计学意义(P<0.05)。治疗后,观察组TIM-1、TIM-3、TLR4蛋白水平均低于参照组,差异有统计学意义(P<0.05)。结论小柴胡汤治疗SLE效果显著,能有效调节免疫指标和血清T细胞免疫球蛋白及黏蛋白水平,改善疾病症状,减少不良反应的发生,值得临床推广应用。
Objective To analyze the clinical efficacy of Xiaochaihu decoction in the treatment of systemic lupus erythematosus(SLE)and its effect on serum immune indexes and levels of T cell immunoglobulin mucin domain.Methods A total of 88 SLE patients admitted to our hospital from April 2018 to August 2020 were selected as the research subjects,and they were divided into the reference group and the observation group by random number table method,with 44 cases in each group.The reference group was treated with rituximab,and the observation group was treated with small bupleurum decoction,the clinical efficacy,incidence of adverse reactions,the levels of T cell immunoglobulin and mucin domain protein molecules(T cell immunoglobulin and mucin domain protein-1[TIM-1],T cell immunoglobulin and mucin domain protein-3[TIM-3],Toll-like receptors 4[TLR4])protein,and serum immune indexes(immunoglobulin A[IgA],immunoglobulin M[IgM],immunoglobulin G[IgG],complement C3,complement C4)were compared between the two groups before and after treatment.Results The total effective rate of treatment in the observation group(95.45%)was higher than that in the reference group(79.55%),and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group(4.55%)was lower than(20.45%)in the reference group,and the difference was statistically significant(P<0.05).After treatment,the levels of IgA,IgM,IgG,and complement C3 in the observation group were lower than those in the reference group,and the level of complement C4 was higher than the reference group,and the difference was statistically significant(P<0.05).Aafter treatment,the levels of TIM-1,TIM-3,and TLR4 protein in the observation group were lower than those in the reference group,and the difference was statistically significant(P<0.05).Conclusion Xiaochaihu decoction has significant effect in the treatment of SLE,can effectively regulate the levels of serum immune index and T cell immunoglobulin and mucin domain,improve disease symptoms,and red
