摘要
目的分析混合痔术后患者医院感染的发生情况及其相关影响因素。方法回顾性分析2016年1月至2021年3月本院普外科收治的498例混合痔患者的临床资料,根据患者术后是否发生医院感染分为感染组(n=29)和未感染组(n=469)。单因素分析混合痔术后患者医院感染发生的危险因素;多因素Logistic回归分析混合痔术后患者医院感染发生的危险因素。结果498例混合痔术后患者中,医院感染发生率为5.82%;单因素分析结果显示,两组年龄、体重指数(BMI)、合并高血压、合并糖尿病、混合痔严重程度、手术时间比较差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,年龄≥60岁、BMI≥25 kg/m^(2)、合并高血压、合并糖尿病、混合痔严重程度为重度、手术时间≥25 min均为混合痔术后患者医院感染发生的独立危险因素(P<0.05)。结论混合痔术后患者医院发生感染与年龄、BMI、合并高血压及糖尿病、混合痔严重程度、手术时间有关,明确发生感染危险因素,可降低混合痔术后患者医院感染发生风险。
Objective To analyze the occurrence of nosocomial infection in patients after mixed hemorrhoids operation and its related influencing factors.Methods The clinical data of 498 patients with mixed hemorrhoids admitted to the general surgery department of our hospital from January 2016 to March 2021 were retrospectively analyzed,and the patients were divided into infected group(n=29)and non-infected group(n=469)according to whether they had nocomial infection after surgery.Univariate analysis of the risk factors of nosocomial infection in patients with mixed hemorrhoids after surgery;multivariate Logistic regression analysis of the risk factors of nosocomial infection in patients after mixed hemorrhoids surgery.Results Among 498 patients with mixed hemorrhoids,the incidence of nosocomial infection was 5.82%;univariate analysis showed that there were statistically significant differences in age,body mass index(BMI),combined with hypertension,combined with diabetes,severity of mixed hemorrhoids and operation time between the two groups(P<0.05).Multivariate Logistic regression analysis showed that age≥60 years old,BMI≥25 kg/m^(2),combined with hypertension,combined with diabetes,severe severity of mixed hemorrhoids,and operation time≥25 min were all independent risk factors for nosocomial infection of patients after mixed hemorrhoids surgery(P<0.05).Conclusion Nosocomial infection in patients with mixed hemorrhoids after surgery was related to age,BMI,combined with hypertension and diabetes,severity of mixed hemorrhoids,and operation time,identifying the risk factors for infection can reduce the risk of nosocomial infection in patients with mixed hemorrhoids after surgery.
