实时三维超声表面与透明成像在胎儿唇腭裂诊断中的应用

摘要 目的探究实时三维超声表面与实时三维透明成像在胎儿唇腭裂诊断中的应用价值。方法回顾性分析2019年9月至2020年10月于本院行中孕期结构畸形筛查的30例唇腭裂胎儿的临床资料,所有孕妇均进行三维超声表面成像与透明成像技术检查,比较两种检查方式的灵敏度、特异度、符合率。结果经产后和引产后的胎儿证实,唇腭裂23例,单纯腭裂3例,单纯唇裂4例。透明成像检查的灵敏度、符合率分别为96.29%、93.33%,均高于表面成像的77.78%、73.33%,差异有统计学意义(P<0.05),两种检查方式特异度比较差异无统计学意义。表面成像检查唇裂时可见胎儿上唇皮肤中断,腭裂检查时胎儿牙槽嵴弓自然弧度消失、出现切迹和/或硬腭部回声中断;透明成像模式检查唇裂表现上唇部出现裂缺、塌鼻;腭裂显示上牙槽的弧形强回声形态不规则、连续性中断和/或半椭圆形略高回声的硬腭内见无回声裂隙。结论透明成像可更直观地显示常规超声难以显示的唇腭裂结构,其超声图像特征明显,能提高胎儿唇腭裂诊断准确率,值得临床推广应用。 Objective To investigate the application of real-time three-dimensional ultrasface and real-time three-dimensional transparency imaging in the diagnosis of fetal cleft lip and palate.Methods The clinical data of 30 fetuses with cleft lip and palate in the screening of structural malformations in themiddle of pregnancy fromSeptember 2019 to October 2020 were retrospectively analyzed,and all pregnant women were examined by three-dimensional ultrasface and transparency imaging technology,the sensitivity,specificity and compliance rate were compared between the two examinationmethods.Results After postpartum and postpartum induction of labor,23 cases of cleft lip and palate,3 cases of cleft palate and 4 cases of cleft lip were confirmed.The sensitivity and compliance rates of transparent imaging examination were 96.29%and 93.33%,respectively,which were higher than 77.78%and 73.33%of three-dimensional ultrasface,and the difference was statistically significant(P<0.05);and there was no significant difference in specificity between the two test modalities.real-time three-dimensional ultrasface shows cleft lip showing the skin of the upper lip of the fetus was interrupted,cleft palate showing the natural arc of the alveolar ridge disappeared,the notch appeared and/or the echo of the hard palate was interrupted;transparent imaging mode shows cleft lip showing upper lip cleft and nose collapse;cleft palate shows an irregular,arc-shaped echo pattern in the upper alveolar,interrupted continuity,and/or non-echoic fissure in the hard palate with slightly higher echo in the upper alveol.Conclusion Transparent imaging can more intuitively display the cleft lip and palate structure that is difficult to show by conventional ultrasound,and its ultrasound image features are obvious,which can improve the accuracy of fetal cleft lip and palate diagnosis,which is worthy of clinical promotion and applation.
