乳腺影像学报告和数据系统分级与彩色多普勒血流成像技术评分应用于乳腺肿瘤定性诊断的价值

摘要 目的探究乳腺影像学报告和数据系统(BIRADS)分级与彩色多普勒血流成像技术(CDFI)评分应用于乳腺肿瘤定性诊断的价值。方法选取2017年5月至2020年6月本院收治的70例乳腺肿瘤患者作为研究对象,所有患者均行超声检查及BIRADS分级与CDFI评分诊断,以病理诊断作为金标准,比较BIRADS分级与CDFI评分应用于乳腺肿瘤定性诊断的灵敏度、特异度、准确度、阳性预测值及阴性预测值。结果病理结果显示,70例乳腺肿瘤患者中恶性病变21例,占30.00%,其中浸润性癌8例,黏液腺癌5例,叶状囊肉瘤5例,髓样癌3例;良性病变49例,占70.00%,其中纤维腺瘤10例,浆细胞乳腺炎12例,纤维脂肪瘤9例,乳头状瘤样增生12例,乳汁淤积性囊肿6例。CDFI评分及BIRADS分级均检出恶性病变23例,良性病变47例,CDFI评分与BIRADS分级灵敏度、特异度、准确度、阳性预测值及阴性预测值比较差异无统计学意义。结论CDFI评分及BIRADS分级均可用于乳腺肿瘤定性的临床初步诊断中,同时,BIRADS分级的规范化及标准化可提高年轻医师的诊断准确度,从而指导临床治疗。 Objective To investigate the value of breast imaging reporting and data system(BIRADS)grading and color Doppler flow imaging(CDFI)score in the qualitative diagnosis of breast tumors.Methods A total of 70 patients with breast tumor admitted to our hospital from May 2017 to June 2020 were selected as the research subjects,and all patients underwent ultrasonography,BIRADS grading and CDFI score for diagnosis,and the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of BIRADS grading and CDFI score in the qualitative diagnosis of breast tumor were compared with pathological diagnosis.Results The pathological results showed that there were 21 cases of malignant lesions in 70 breast tumor patients,accounting for 30.00%,including 8 cases of invasive carcinoma,5 cases of mucinous adenocarcinoma,5 cases of leaf cyst sarcoma and 3 cases of medullary carcinoma;there were 49 cases of benign lesions,accounting for 70.00%,including 10 cases of fibroadenoma,12 cases of plasma cell mastitis,9 cases of fibrolipoma,12 cases of papilloma-like hyperplasia,and 6 cases of milk stasis cyst.23 cases of malignant lesions and 47 cases of benign lesions were detected in CDFI score and BIRADS grade,and there was no significant difference in the sensitivity,specificity,accuracy,positive predictive value and negative predictive value between CDFI score and BIRADS grade.Conclusion Both CDFI score and BIRADS grading can be used in the qualitative preliminary clinical diagnosis of breast tumors,and the standardization and standardization of BIRADS grading can improve the diagnostic accuracy of young doctors,so as to guide clinical treatment.
