摘要
目的探讨老年肾结石患者的结石成分及结石分析的应用价值。方法回顾性分析2017年3月至2019年1月本院收治的78例老年肾结石患者的临床资料。比较单一成分结石与混合型结石占比;分析结石成分构成比例及复发情况。结果结石成分单一患者占比为24.36%,低于混合型结石75.64%,差异有统计学意义(P<0.05)。78例老年肾结石患者中,碳酸磷灰石结石27例,二水草酸钙结者15例,一水草酸钙结石14例,六水磷酸铵镁结石8例,无水尿酸结石7例,尿酸铵结石3例,二水磷酸氢钙结石3例,一水尿酸钠结石1例;结石成分主要为碳酸磷灰石、二水草酸钙、一水草酸钙占比71.79%。随访1年,78例老年肾结石患者中,脱落3例,复发7例,复发率为12.82%。结论老年肾结石多为混合型结石,以碳酸磷灰石、二水草酸钙、一水草酸钙为主要成分,分析结石成分为肾结石患者的治疗、护理提供参考,有助于制订相应的防控措施,防止患者远期复发,值得临床推广应用。
Objective To investigate the clinical value of stone composition and stone analysis in elderly patients with renal calculi.Methods The clinical data of 78 elderly patients with renal calculi admitted to our hospital from March 2017 to January 2019 were retrospectively analyzed.The proportion of single component stones and mixed stones was compared,the proportion of stone composition and recurrence were analyzed.Results The proportion of patients with single stone composition was 24.36%,which was lower than that of mixed stones(75.64%),and the difference was statistically significant(P<0.05).Among the 78 elderly patients with kidney stones,there were 27 cases of carbonate apatite stones,15 cases of calcium oxalate dihydrate stones,14 cases of calcium oxalate monohydrate stones,8 cases of magnesium ammonium phosphate hexahydrate stones,7 cases of anhydrous uric acid stones,3 cases of ammonium urate stones,3 cases of calcium hydrogen phosphate dihydrate stones,and 1 case of sodium urate monohydrate stones;The main stone components were carbonate apatite,calcium oxalate dihydrate,and calcium oxalate monohydrate,accounting for 71.79%.After 1 year of follow-up,among 78 elderly patients with kidney stones,3 cases dropped off and 7 cases recurred,with the recurrence rate of 12.82%.Conclusion Most of the elderly kidney stones are mixed stones,with carbonate apatite,calcium oxalate dihydrate and calcium oxalate monohydrate as the main components.Analysis of stone composition provides a reference for the treatment and nursing of patients with kidney stones,helps to formulate corresponding prevention and control measures,and prevents long-term recurrence of patients,which is worthy of clinical promotion and application.
