摘要
目的探讨多种西药合用不良反应及合理用药的意义。方法回顾性分析2019年4月至2021年3月本院门诊开具的1200份多种西药合用处方,分析多种西药合用后发生不良反应的原因、种类,统计多种西药合用的不合理配伍情况。结果1200份多种西药合用处方中,出现药物不良反应的处方共200份,不良反应发生率为16.67%;多种西药合用后发生不良反应的原因中,不良反应发生原因有4种,依次为不良配伍、重复给药、用法不当、剂量不当。药物不良反应10种,依次为过敏反应、呕吐、肢体乏力、头晕、头痛、局部水肿、全身发热、恶心、皮肤反应、睡眠质量下降。不合理配伍情况有5种,依次为阿莫西林+炔雌醇环丙孕酮片、抗菌药物配伍不当、阿奇霉素片+抗酸药物、氨茶碱+普萘洛尔、抗菌药物+注射用盐酸氨溴索。结论多种西药合用引发的不合理用药现象严重威胁患者的身心健康及生命安全,临床应优化处方审核监管流程,规范多种西药合用处方,正确指导患者用药,从而确保用药有效性及安全性。
Objective To investigate the adverse reactions of multiple western drugs combined and the significance of rational drug use.Methods A retrospective analysis was performed on 1200 prescriptions of multiple western drugs in the outpatient department of our hospital from April 2019 to March 2021,the causes and types of adverse reactions after the combination of a variety of western drugs were analyzed,and the unreasonable compatibility of a variety of western drugs was counted.Results Among the 1200 prescriptions of multiple western medicines,200 prescriptions had adverse drug reactions,and the incidence of adverse drug reactions was 16.67%.Among the causes of adverse reactions after the combination of a variety of western drugs,there were 4 causes of adverse reactions,including poor compatibility,repeated administration,improper usage and improper dosage.There were 10 kinds of adverse drug reactions,including allergic reaction,vomiting,limb weakness,dizziness,headache,local edema,systemic fever,nausea,skin reaction,and decreased sleep quality.There were 5 kinds of unreasonable compatibility,including amoxicillin+ethinylestradiol and cyproterone tablets,inappropriate compatibility of antibacterial drugs,azithromycin tablets+antacid drugs,aminophylline+propranolol,antibacterial drugs+ambroxol hydrochloride for injection.Conclusion Irrational drug use caused by the combination of multiple western drugs seriously threatens the physical and mental health and life safety of patients.In clinical practice,we should optimize the prescription audit and supervision process,standardize the combination of multiple western drugs,and correctly guide patients to use drugs,so as to ensure the effectiveness and safety of drug use.
