双模式人工耳蜗植入者临床疗效Meta分析 被引量:1

摘要 目的通过比较双模式(CI+HA)与单耳人工耳蜗植入模式(CI)的临床效果,明确双模式能否有效提高人工耳蜗植入术后听觉及言语能力。方法检索PubMed、Cochrane、CBM、CNKI、维普、万方数据库,收集2022年5月前国内外公开发表的关于人工耳蜗植入术后双模式与单耳模式康复疗效比较的相关文献,对符合纳入标准的文献资料进行Meta分析。结果共纳入9篇文献,术后采用言语可懂度分级标准(speech intelligibility rating,SIR)、单/双音节词言语识别率、安静/噪声条件下言语识别率进行听力和言语评估。CI+HA组和CI组耳聋患者人工耳蜗植入术后SIR、单音节词、安静短句、噪声短句的效果差异分别为0.34(0.19,0.50)、3.28(0.19,6.37)、4.25(1.08,7.42)、11.8(5.42,18.18),差异有统计学意义(P<0.05)。结论与单耳人工耳蜗模式相比,双模式能够更有效提高人工耳蜗植入者的听觉及言语能力。 Objective To compare the clinical outcomes of bimodal hearing(CI+HA)and monaural cochlear implant hearing(CI),and to determine whether bimodal hearing mode can effectively improve the speech and hearing ability after cochlear implantation.Methods PubMed,Cochrane,CBM,CNKI,Weipu and Wanfang databases were searched to identify local and international studies published by May 2022 with outcomes of CI+HA group and CI group after cochlear implantation.Meta-analysis of literature materials meeting the inclusion criteria was conducted.Results A total of 9 articles were included for the meta-analysis.Speech intelligibility rating(SIR),speech recognition scores of monosyllabic words,disyllabic words and sentences under quiet and noise conditions were used to evaluate the hearing and speech performance after implantation.The differences between CI+HA group and CI group in SIR,monosyllabic words,sentences in quiet and noise were 0.34[95%CI(0.19,0.50)],3.28[95%CI(0.19,6.37)],4.25[95%CI(1.08,7.42)],and 11.8[95%CI(5.42,18.18)],respectively,with significant statistical differences between the two groups(P<0.05).Conclusion Comparing with monaural cochlear implant hearing,bimodal hearing can effectively improve hearing and speech ability in cochlear implant recipients.
