摘要
目的:探讨内镜下钛夹联合药物治疗非静脉曲张性上消化道出血的效果。方法:选取2020年4月—2022年4月钦州市第二人民医院收治的80例非静脉曲张性上消化道出血患者作为研究对象,采用随机数字表法分为观察组与对照组,各40例。对照组采取内镜下药物黏膜注射法治疗,观察组在对照组基础上采取内镜下钛夹治疗。比较两组止血效果、炎性因子水平、不良反应发生情况。结果:观察组止血总有效率高于对照组,差异有统计学意义(P=0.0122)。治疗前,两组白细胞介素-6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平比较,差异无统计学意义(P>0.05);治疗后,两组IL-6、CRP、TNF-α水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组不良反应总发生率低于对照组,差异有统计学意义(P=0.0338)。结论:内镜下钛夹联合药物治疗非静脉曲张性上消化道出血兼具安全性、有效性,止血效果好且炎性反应轻微。
Objective:To explore the effect of endoscopic titanium clips combined with drugs in the treatment of non-variceal upper gastrointestinal bleeding.Methods:A total of 80 patients with non-variceal upper gastrointestinal bleeding admitted to the Second People’s Hospital of Qinzhou from April 2020 to April 2022 were selected as the study subjects.According to the method of random number table,they were divided into the control group and the observation group,with 40 cases in each group.The control group was treated by endoscopic drug mucosal injection and and the observation group was treated by endoscopic titanium clips on the basis of control group.The hemostatic effect,inflammatory factor levels,and occurrence of adverse reactions between the two groups were compared.Results:The total effective rate of hemostasis in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.0122).Before treatment,there was no statistically significant difference in the levels of interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factor-α(TNF-α)between the two groups(P>0.05).After treatment,the levels of IL-6,CRP and TNF-αin the two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.0338).Conclusion:Endoscopic titanium clips combined with drugs are both safe and effective in the treatment of non-variceal upper gastrointestinal bleeding.The hemostatic effect is good,and the inflammatory reaction is mild.
