摘要
目的探讨全膝关节置换(TKA)患者出院准备服务需求情况及影响因素,为制定出院准备服务提供科学依据。方法采用TKA患者出院准备服务需求调查表,对2022年1—5月河南省洛阳正骨医院(河南省骨科医院)郑州院区108例行TKA的患者在其出院前发放问卷进行横断面调查与分析,了解患者的出院准备服务需求,包括膝关节疾病/置换相关知识宣教、指导如何处理关节疼痛等14个条目。结果TKA患者出院准备服务需求得分为21~70分,中位得分为34.00(29.00,43.00)分,排在前4位的分别为办理报销、病例复印等事宜的指导、康复锻炼的指导、办理出院或转院流程的指导和并发症的护理指导。多元线性逐步回归分析结果显示,患者的婚姻状况、住院期间照顾者类型、膝关节病程是TKA患者出院准备服务需求的影响因素(R^(2)=0.363,调整R^(2)=0.290,F=4.974,P<0.001)。结论TKA患者对出院准备服务的需求处于中高等水平,患者的婚姻状况、住院期间照顾者类型、膝关节病程是TKA患者出院准备服务需求的影响因素,医护人员应以需求为导向,充分评估患者的社会支持等情况,制定并实施相应的出院准备服务,满足患者需求,提高出院准备水平,促进康复。
Objective To investigate the demand and influencing factors of discharge preparation service for patients with total knee replacement(TKA),and to provide scientific basis for the development of hospital discharge preparation service.Methods A cross-sectional survey and analysis were conducted on 108 patients undergoing TKA in Luoyang Orthopedic Hospital of Henan Province(Orthopedic Hospital of Henan Province)from January to May 2022 in order to understand their demands for discharge preparation services by using the“Questionnaire on service requirements for discharge preparation of TKA patients”.There are 14 items including knee disease/replacement related knowledge education and guidance on how to deal with joint pain.Results The service demand scores of TKA patients ranged from 21 to 70 points,the median score was 34.00(29.00,43.00)points.The top 4 were the guidance for handling reimbursement,case copy and other matters,the guidance for rehabilitation exercise,the guidance for discharge or transfer process and the nursing guidance for complications.Multiple linear stepwise regression analysis results showed that the patient’s marital status,the type of caregivers during hospitalization and the course of knee joint disease were the factors influencing the service demand of TKA patients(R^(2)=0.363,the adjusted R^(2)=0.290,F=4.974,P<0.001).Conclusion The demand for discharge preparation services of TKA patients is at a medium and high level,and the marital status of patients,the type of caregivers during hospitalization and the course of knee joint disease are the factors affecting the demand for discharge preparation services of TKA patients.Medical staff should be demand-oriented,develop and implement corresponding discharge preparation services,so as to meet patients’needs,improve discharge preparation and promote rehabilitation.
