摘要
脱发是临床常见疾病之一,主要特征是头部毛发进行性减少或成片脱落,中医学称之为“虫蛀脱发”或“油风”。临床脱发的类型较多,主要包括雄激素性脱发、斑秃、化疗性脱发、老年性脱发、物理性脱发及瘢痕性脱发等多种。脱发给人们生活带来诸多不便,令患者非常苦恼。临床上比较常见的脱发疾病是脂溢性脱发和斑秃。当前认为脂溢性脱发的发病与遗传、内分泌失调、精神压力等因素有关,斑秃则可能与遗传、情绪应激、自身免疫有关。中医认为头发的生长与血气、肝肾密切相关,且头皮容易受到风邪和湿邪的侵袭,在临床中,如果患者出现气血不和,肝肾精血不足或者头皮受到风邪、湿邪等侵袭,容易导致头发脱落。文章从中医角度提出脱发疾病的四大主要诱发因素,即气血不和、风邪侵袭、肾精不足、湿热羁恋,并针对这四种病因分别予以对症治疗,即调理气血、祛除风邪、补肾益精、清利湿热。临床应标本兼顾,灵活变通,方可有效消除临床症状,从而解决脱发难题。
Alopecia is one of the common clinical diseases,which is mainly characterized by progressive reduction of head hair or shedding in pieces,which is called"moth-eaten alopecia"or"alopecia areata"in traditional Chinese medicine.There are many types of clinical alopecia,including androgenic alopecia,alopecia areata,chemotherapy alopecia,senile alopecia,physicalA alopecia and cicatricial alopecia.Alopecia brings a lot of inconvenience to people’s life,making patients very distressed.The common alopecia is diseases in clinic are seborrheic alopecia and alopecia areata.At present,it is believed that the pathogenesis of seborrheic alopecia is related to genetics,endocrine disorders,mental stress and so on,while alopecia areata may be related to genetics,emotional stress and autoimmunity.Traditional Chinese medicine believes that the growth of hair is closely related to blood Qi,liver and kidney,and the scalp is easy to be attacked by wind and dampness.In clinic,if patients have discord between Qi and blood,deficiency of liver and kidney or are invaded by wind and dampness,it is easy to cause hair loss.From the perspective of TCM,four main inducing factors of alopecia are proposed in this paper,namely,disharmony of Qi(气)and blood,invasion of pathogenic wind,deficiency of kidney essence,and obstruction with damp and heat.According to the above four causes,four symptomatic therapies are given,respectively,namely regulating Qi and blood,removing pathogenic wind,tonifying kidney and nourishing essence,and clearing away dampness and heat.In order to effectively eliminate clinical symptoms,the treatment of this disease should be flexible,taking into account treating both principal and secondary aspect of diseases.
