基质细胞衍生因子-1α联合富血小板纤维蛋白在种植牙骨组织再生中的应用研究 被引量:1

摘要 目的探讨基质细胞衍生因子-1α联合富血小板纤维蛋白,在促进种植牙牙槽骨组织再生过程中的作用。方法以2019年6月至2020年12月收治的120例种植牙患者为研究对象,根据随机数字表法分为实验组(n=60)和对照组(n=60)。对照组采用富血小板纤维蛋白+人工骨粉,实验组采用基质细胞衍生因子-1α+富血小板纤维蛋白+人工骨粉。对比两组患者的骨密度、骨厚度、牙槽骨唇舌向宽度、牙龈健康情况、种植牙成功率、完全愈合率、局部感染率、种植体稳定系数(ISQ)、探诊深度(PD)、龈沟出血指数(SBI);术后7 d,酶联免疫吸附试验和Western blot检测龈沟液和骨屑中血小板衍生生长因子(PDGF)、转化生长因子-β(TGF-β)、骨形态发生蛋白2(BMP-2)表达水平。结果术后7 d,实验组PDGF、TGF-β、BMP-2蛋白表达水平高于对照组(P<0.05)。术后6个月,实验组骨密度、骨厚度、牙槽骨唇舌向宽度、种植体稳定系数高于对照组(P<0.05),探诊深度和龈沟出血指数低于对照组(P<0.05);两组患者种植牙成功率、完全愈合率和局部感染率对比,差异无统计学意义(P>0.05)。结论基质细胞衍生因子-1α联合富血小板纤维蛋白可以促进种植牙患者牙槽骨组织再生修复,相关作用机制可能与上调PDGF、TGF-β和BMP-2表达有关。 Objective To investigate the effect of stromal cell derived factor-1αcombined with platelet-rich fibrin on alveolar bone regeneration after dental implantation.Methods A total of 120 dental implant patients were selected from June 2019 to December 2020.According to random number table method,patients were divided into experimental group(n=60)and control group(n=60).The control group was treated with platelet-rich fibrin+artificial bone meal,and the experimental group was treated with stromal cell-derived factor-1α+platelet-rich fibrin+artificial bone meal.Bone mineral density,bone thickness,alveolar bone lip and tongue width,gingival health,implant success rate,complete healing rate,local infection rate,implant stability coefficient(ISQ),probing depth(PD)and gingival crevicular bleeding index(SBI)were compared between the two groups.Enzyme-linked immunosorbent assay and Western blot was used to detect the expression levels of platelet-derived growth factor(PDGF),transforming growth factor(TGF-β)and bone morphogenetic protein 2(BMP-2)in gingival crevicular fluid and bone chips 7 days after operation.Results The expression levels of PDGF,TGF-βand BMP-2 protein in the experimental group were higher than those in the control group on the 7th day after surgery(P<0.05).The bone density,bone thickness,alveolar bone lip-lingual width and implant stability coefficient in the experimental group were higher than those in the control group at the 6th month after operation(P<0.05).The probing depth and gingival crevicular bleeding index were lower than those in the control group(P<0.05).There was no significant difference in the implant success rate,complete healing rate and local infection rate between the two groups 6 months after surgery(P>0.05).Conclusion Stromal cell derived factor-1αcombined with platelet-rich fibrin can promote alveolar bone regeneration and repair in patients with dental implants,and the relevant mechanism may be related to the up-regulation of PDGF,TGF-βand BMP-2 expression.
