摘要
目的探讨乳腺癌3.0 T MRI影像学特征与Ki-67表达的相关性。方法选取2020年1月~2021年12月216例经手术切除病理证实乳腺癌患者作为研究对象,所有患者治疗前均行常规3.0 T MRI、扩散加权成像(DWI)及动态增强(DCE)检查,对比分析乳腺癌MRI影像特征、表观扩散系数(ADC)值及时间-信号曲线(TIC)与Ki-67的表达情况。结果216例乳腺癌中Luminal A型26例(12%),Luminal B型121例(56%),HER2过表达型46例(21%),三阴性乳腺癌23例(11%)。Ki-67低表达37例,高表达179例;肿瘤最大直径、有无坏死、有无瘤周水肿、有无腋窝淋巴结转移、有无血管增多增粗、TIC曲线类型、ADC值在Ki-67高、低表达组的差异均有统计学意义(P<0.05);肿瘤是否肿块型、是否边界清晰在Ki-67高、低表达组的差异无统计学意义(P>0.05)。多因素Logistics回归模型分析,模型1、模型2、模型3结果显示ADC值(OR=4.27、4.20、7.82,P=0.001、0.002、<0.001)和瘤周水肿(OR=4.42、4.52、6.00,P=0.004、0.004、0.002)均与Ki-67表达有关。结论乳腺癌MRI影像学特征对于预测肿瘤增殖指数Ki-67表达有重要意义,ADC值、瘤周水肿可作为Ki-67表达独立预测因素。
Objective To evaluate the correlation between 3.0 T MRI features and the Ki-67 expression in breast cancer.Methods Between January 2020 to December 2021,a total of 216 patients with histologically confirmed breast cancer underwent routine 3.0 T MRI,diffusion-weighted imaging(DWI)and dynamic contrast-enhanced(DCE)scan before treatment.The MRI features,apparent diffusion coefficient(ADC)value,enhancement time-signal curve(TIC),and Ki-67 expression of breast cancer were compared and analyzed.Results The 216 breast cancer included 26(12%)luminal A,121(56%)luminal B,46(21%)HER2 over expression,and 11% triple negative breast cancer.37 tumors had low Ki-67 expression and 179 had high expression were Ki-67.There were significant differences in the maximum diameter of the tumor,the presence or absence of necrosis,peritumoral edema,axillary lymph node metastasis,increased blood vessels,TIC type,and ADC value between the Ki-67 high and low expression groups(P<0.05).There was no significant difference in tumor morphology and tumor margin between Ki-67 high and low expression groups(P>0.05).Multivariate logistic regression model analysis showed that ADC value(OR=4.27,4.20,7.82;P=0.001,0.002,<0.001)and peritumoral edema(OR=4.42,4.52,6.00;P=0.004,0.004,0.002)were related to Ki-67 expression in model 1,model 2,and model 3.Conclusion MRI features of breast cancer are important for predicting the expression of Ki-67.The ADC value and peritumoral edema can be used as independent predictors of Ki-67 expression.
