Effect of Family-Centered Health Education on Psychological Status and Treatment Compliance in Children with Epilepsy

摘要 目的:探讨以家庭为中心的健康教育对癫痫患儿心理状况及遵医行为的影响。方法:选取2019年6月1日~2021年6月30日诊治的180例癫痫患儿,采用简单随机分组法分为对照组和观察组各90例,对照组采用常规健康教育,观察组采用以家庭为中心的健康教育;比较两组患儿教育前后心理状况[采用儿童抑郁障碍自评量表(DSRSC)和儿童焦虑性情绪障碍筛查量表(SCARED)]、羞耻感(采用羞耻感量表)、生活质量[采用癫痫患儿生活质量评定量表(QOLCE)]、遵医行为,比较两组家长教育前后疾病管理能力[采用家庭管理测量量表(FAMM)]。结果:干预后,两组患儿DSRSC、SCARED评分、羞耻感量表各维度得分均低于干预前(P<0.05)、QOLCE中各维度得分均高于干预前(P<0.05),且观察组优于对照组(P<0.01);观察组患儿遵医率高于对照组(P<0.01);干预后,两组家长FAMM中疾病照护能力得分高于干预前(P<0.05)、疾病管理困难得分低于干预前(P<0.05),且观察组优于对照组(P<0.05,P<0.01)。结论:以家庭为中心的健康教育可改善癫痫患儿的心理状况和生活质量,提高患儿遵医行为和家长疾病管理能力,减轻患儿羞耻感。 Objective:To explore the effect of family-centered health education on psychological status and treatment compliance in children with epilepsy.Methods:A total of 180 children with epilepsy treated were enrolled between June 1,2019 and June 1,2021,and according to the simple random grouping method were divided into the control group and the observation group with 90 cases in each group.The control group was given routine health education,while the observation group was given family-centered health education.The psychological status by depression self-rating scale for children(DSRSC),screen for child anxiety related emotional disorders(SCARED),stigma(stigma scale),quality of life by quality of life for children with epilepsy(QOLCE),treatment compliance,and disease management ability of children′family members by family management measure(FAMM)between the two groups were compared before and after education.Results:After intervention,the scores of DSRSC,SCARED and stigma were lower in both groups than those before intervention(P<0.05),and QOLCE scores were higher in both groups than those before intervention(P<0.05),and QOLCE scores were higher in the observation group than those in the control group(P<0.01).The treatment compliance rate in the observation group was higher than that in the control group(P<0.01).After intervention,scores of family members′disease care ability were higher in both groups than those befire intervention(P<0.05),and scores of disease management difficulty were lower in both groups than those befire intervention(P<0.05),and scores of disease management difficulty were lower in the observation group than those in the control group(P<0.05,P<0.01).Conclusion:Family-centered health education can improve psychological status and quality of life in epilepsy children,improve treatment compliance and disease management ability of family members,and alleviate stigma.
