摘要
目的挖掘《伤寒瘟疫条辨》的遣方用药规律,探讨杨栗山治疗温病的学术思想,为临床诊治提供参考。方法收集《伤寒瘟疫条辨》涉及的处方,采用Excel2010构建方药数据库,依据IBM SPSS Statistics 26.0和IBM SPSS Modeler 14.2软件对纳入的方药进行频次、因子、关联、系统聚类分析,探究组方规律。结果经筛选,纳入181首处方、涉及188味药物。高频药物有甘草、生姜、白芍、黄芩、大枣、桂枝、人参、黄连、当归、大黄等;以寒、温性,苦、辛、甘味居多,多入肺、脾、胃经。功效分类多集中于补虚药、清热药、解表药。关联分析得到的药物组合有黄连-栀子-黄芩、大枣-白芍-生姜、当归-地黄等。主成分分析得到11组公因子,系统聚类得到8组聚类组合。结论杨栗山以杂气怫郁三焦立论,以“轻者清之,重者降之”治疗温病,重视表里双解的治法。以甘寒之品化生津血、苦寒之品涤荡浊邪、辛凉之品透表养营,甘寒之品不易运化,可稍佐温药温通。
Objective To explore the medication law of prescriptions in Shang Han Wen Yi Tiao Bian;To explore Yang Lishan’s academic thought on treating warm pathogen disease;To provide reference for clinical diagnosis and treatment.Methods The prescriptions in Shang Han Wen Yi Tiao Bian were collected,and the database of prescriptions was established by Excel 2010.According to the software of IBM SPSS Statistics 26.0 and IBM SPSS Modeler 14.2,the prescription law was analyzed by using the techniques of frequency,factor,correlation and systematic clustering.Results After screening,181 prescriptions involving 188 kinds of Chinese materia medica were included.High-frequency drugs include Glycyrrhizae Radix et Rhizoma,Zingiberis Rhizoma Recens,Paeoniae Radix alba,Scutellariae Radix,Jujubae Fructus,Cinnamomi Ramulus,Ginseng Radix et Rhizoma,Coptidis Rhizoma,Angelicae Sinensis Radix,Rhei Radix et Rhizoma,etc.;the efficacy was mostly concentrated in tonic drugs,heatclearing drugs and exterior-relieving drugs.The main properties were cold and warm,and the main tastes were bitter,pungent and sweet,mostly entering the lung,spleen and stomach meridians.The drug combinations obtained by correlation analysis included Coptidis Rhizoma,-Gardenia Fructus-Scutellariae Radix,Jujube Fructus-Paeoniae Radix alba-Ginseng Radix et Rhizoma,Angelicae Sinensis Radix-Rehmanniae Radix.11 groups of common factors were obtained by principal component analysis,and 8 groups of clustering combinations were obtained by systematic clustering.Conclusion Yang Lishan’s theory is based on miscellaneous qi and depressed triple energizer,and he treats warm pathogen disease with“clearing it from the light and descending it from the heavy”,and attaches importance to the treatment of both exterior and interior solutions.Sweet and cold products are used to produce body fluid and blood,bitter and cold products are used to cleanse turbid pathogens,and pungent and cool products are used to nourish the nutrition.Sweet and cold products are not easy to transport,an
