摘要
目的:探讨抚触结合中医穴位按摩对光疗高胆红素血症(HB)患儿护理效果的影响。方法:选取2020年10月至2021年12月徐州市贾汪区人民医院新生儿监护室收治HB患儿100例,随机分组后分别给予常规干预(光疗以及补水、皮肤护理和提供饮食指导等)和常规干预、抚触结合穴位按摩,比较两组干预后黄疸持续时间、胎便转黄时间、血清总胆红素水平、心理运动功能相关评分和光疗反应发生情况。结果:观察组黄疸持续(6.09±1.44)h、胎便转黄时间(33.42±1.75)h均短于对照组,血清总胆红素水平(86.14±15.69)μmol/L低于对照组(P<0.05)。干预4周后观察组心理运动功能相关评分高于对照组(P<0.05),光疗反应总发生率(26.67%vs 48.33%)低于对照组(P<0.05)。结论:抚触结合中医穴位按摩可缩短HB患儿黄疸持续和胎便转黄时间,降低血清总胆红素水平,改善心理运动功能,减少光疗反应,提高护理满意度,效果显著。
Objective:To explore the effect of massage combined with TCM acupoint massage on the nursing effect of phototherapy on hyperbilirubinemiain children.Methods:Selected 100 children with HB who were admitted to the neonatal intensive care unit of Jiawang District People's Hospital of Xuzhou City from October 2020 to December 2021.After randomization,they were given routine interventions(phototherapy,hydration,skin care,and dietary guidance,etc.)and routine interventions.Intervention,stroking and acupoint massage were used to compare the duration of jaundice,time to yellow meconium,serum total bilirubin level,psychomotor function-related scores and phototherapy response between the two groups after intervention.Results:The duration of jaundice in the observation group(6.09±1.44)h was shorter than that in the control group(33.42±1.75)h.The serum total bilirubin level(86.14±15.69)μmol/L was lower than that of control group(P<0.05).After 4 weeks of intervention,the correlation score of psychomotor function in the observation group was higher than that of the control group(P<0.05),the total incidence of phototherapy reaction(26.67%vs48.33%)was lower than that of control group(P<0.05).Conclusion:Massage combined with TCM acupoint massage can shorten the duration of jaundice and fecal yellowing in HB children,reduce the level of serum total bilirubin,improve psychomotor function,reduce phototherapy response,improve nursing satisfaction,nursing effect is significant.
