摘要
目的观察米非司酮联合甲氨蝶呤治疗异位妊娠(EP)的临床效果及对患者血清生化指标、凝血功能的影响。方法按治疗方式的不同将本院2019年1月至2020年12月收治的50例EP患者分为对照组和观察组,各25例。对照组以甲氨蝶呤治疗,观察组以米非司酮联合甲氨蝶呤治疗。比较两组的临床症状、血清生化指标、凝血功能指标及生活质量。结果观察组的腹痛消失、盆腔积液吸收、包块消失、血β-人绒毛膜促性腺激素(β-hCG)转阴、月经复潮时间均短于对照组(P<0.05)。治疗后,两组的β-hCG、抑制素A(INH-A)、糖类抗原125(CA125)、血管内皮生长因子(VEGF)水平均降低,且观察组低于对照组(P<0.05)。治疗后,两组的活化部分凝血活酶时间(APTT)缩短,纤维蛋白原(FIB)水平升高,血小板计数(PLT)降低,且观察组优于对照组(P<0.05)。治疗后3个月,两组的女性性功能指数(FSFI)、生活质量综合评定问卷(GQOLI-74)评分均升高,且观察组显著高于对照组(P<0.05)。结论米非司酮联合甲氨蝶呤治疗EP可促进患者临床症状恢复,改善血清生化指标及凝血功能,提高生活质量,值得临床应用。
Objective To observe the clinical effect of mifepristone combined with methotrexate in the treatment of ectopic pregnancy(EP)and its influences on serum biochemical indexes and coagulation function of patients.Methods According to different treatment methods,50 EP patients admitted in our hospital from January 2019 to December 2020 were divided into control group and observation group,with 25 cases in each group.The control group was treated with methotrexate,and the observation group was treated with mifepristone combined with methotrexate.The clinical symptoms,serum biochemical indexes,coagulation function indexes and quality of life were compared between the two groups.Results The time of disappearance of abdominal pain,pelvic effusion absorption,mass disappearance,bloodβ-human chorionic gonadotropin(β-hCG)negative conversion and menstrual recovery in the observation group were shorter than those in the control group(P<0.05).After treatment,the levels ofβ-hCG,inhibin-A(INH-A),carbohydrate antigen 125(CA125)and vascular endothelial growth factor(VEGF)in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,the activated partial thromboplastin time(APTT)of the two groups shortened,the fibrinogen(FIB)level increased,and the platelet count(PLT)decreased,and those in the observation group were better than the control group(P<0.05).At 3 months after treatment,the scores of Female Sexual Function Index(FSFI)and Generic Quality of Life Inventory-74(GQOLI-74)in the two groups increased,and those in the observation group were significantly higher than the control group(P<0.05).Conclusion Mifepristone combined with methotrexate in the treatment of EP can promote the recovery of clinical symptoms,improve serum biochemical indexes and coagulation function,and improve the quality of life,which is worthy of clinical application.
