摘要
目的 探讨宫颈不成熟的足月妊娠阴道分娩初产妇适合的引产方式。方法 回顾性分析2018年6月—2022年6月有引产指征在巩义瑞康医院引产的足月妊娠阴道分娩初产妇,宫颈Bishop评分<6分。其中,应用地诺前列酮栓75例,简称药物组;应用Foley导尿管球囊联合人工破膜、静滴缩宫素65例(包含10例有一次子宫手术史者),简称球囊组。对比两组宫颈Bishop评分、促宫颈成熟引产成功率、阴道分娩率、转剖宫产率、住院天数及母婴安全性。结果 药物组引产成功率97.33%,球囊组引产成功率95.38%,两组引产成功率无明显差异。两组在母婴结局上比较无明显差异:产后出血率、新生儿窒息率相当,均无产褥感染及其他严重并发症发生。母婴结局良好。结论 两者均为有效的促宫颈成熟引产方式。临床工作中要详细告知患者两者的风险与差别、结合患者宫颈Bishop评分、对住院待产时间、疼痛耐受等情况的接受程度,综合评估,选择合适的促宫颈成熟引产方式。
Objective To explore the method of induction of labor for primipara with vaginal delivery and full-term pregnancy with immature cervix.Methods From June 2018 to June 2022,the full-term pregnant primiparas who had the indication of induction of labor in our hospital were analyzed retrospectively,and the cervix Bishop score was less than 6 points.Among them,75 cases were treated with dinoprostone suppository(drug group for short);Foley catheter balloon combined with artificial rupture of membrane and intravenous oxytocin were used in 65 cases(inclu⁃ding 10 cases with a history of uterine surgery),referred to as balloon group.The Bishop score of cervix,the success rate of inducing labor by promoting cervical prematurity,the rate of delivery through the vagina,the rate of cesarean section,the number of days in hospital and the safety of mothers and infants were compared between the two groups.Results The success rate of labor induction in the drug group was 97.33%,and that in the balloon group was 95.38%.There was no significant difference between the two groups.There was no significant difference in maternal and infant outcome between the two groups:the rate of postpartum hemorrhage and neonatal asphyxia were similar,and there was no puerperal infection and other serious complications.Maternal and infant outcomes were good.Conclusion Both of them are effective methods to promote cervical maturation and induce labor.In clinical work,pa⁃tients should be informed in detail of the risks and differences between the two,combined with the patient’s cervix Bishop score,acceptance of the hospital waiting time for labor,pain tolerance and other conditions,evalute comprehen⁃sively,and select the appropriate method of indcing labor by promoting cervical maturity.
