摘要
目的:探讨艾愈胶囊联合同步放化疗对经TPF(多西他赛+顺铂+氟尿嘧啶)诱导化疗后局部晚期鼻咽癌患者的疗效、中医证候评分、不良反应、免疫功能和预后的影响。方法:收治的140例局部晚期鼻咽癌患者,采用随机数字表法将患者分为两组,两组均先给予TPF诱导化疗后,观察组给予艾愈胶囊联合同期放化疗,对照组给予单纯同期放化疗。比较两组疗效、中医证候评分、不良反应、免疫功能和预后。治疗完成后随访至2022年2月,比较两组生存率差异。结果:共130例最终完成治疗,每组各65例。观察组疾病控制率(DCR)高于对照组(P<0.05),观察组放疗中断比例、Ⅲ度及以上口腔黏膜炎、白细胞减少、重度贫血、恶心呕吐、等不良反应的发生率低于对照组(P<0.05),同步完成率高于对照组(P<0.05)。与治疗前比较,两组治疗后CD3^(+)T细胞、CD4^(+)T细胞、CD4^(+)/CD8^(+)、CD3-CD56^(+)NK细胞、CD56brightNK细胞、CD56dimNK细胞、CD39^(+)CD73^(+)B细胞、CD19^(+)B细胞、Th17、Th17/Treg升高(P<0.05),中医证候积分、Treg细胞、CD8^(+)T细胞降低(P<0.05);治疗后,观察组CD3^(+)T细胞、CD4^(+)T细胞、CD4^(+)/CD8^(+)、CD3-CD56^(+)NK细胞、CD56 brightNK细胞、CD56 dimNK细胞、CD39^(+)CD73^(+)B细胞、CD19^(+)B细胞、Th17、Th17/Treg高于对照组(P<0.05),中医证候积分、Treg细胞、CD8^(+)T细胞低于对照组(P<0.05);与治疗前比较,两组治疗后白细胞计数明显减少(P<0.05),治疗后观察组白细胞计数明显高于对照组(P<0.05)。中位随访15(12~27)个月,130例患者失访9例,观察组疾病控制率(DCR)高于对照组(P<0.05),PFS、OS生存率明显高于对照组(P<0.05)。结论:艾愈胶囊联合同步放化疗可改善TPF诱导化疗后局部晚期鼻咽癌患者免疫功能及中医证候评分,降低放化疗相关不良反应发生率、放疗中断率,提高同步完成率、疗效,并改善患者生存状况。
Objective:To investigate the effects of Aiyu(艾愈)Capsules combined with concurrent chemoradiotherapy on therapeutic efficacy,traditional Chinese medicine(TCM)syndrome score,adverse reactions,cellular immunity,and prognosis in patients with locally advanced nasopharyngeal carcinoma(NPC)after TPF(docetaxel+cisplatin+fluorouracil)-induced chemotherapy.Methods:A total of 140 patients with locally advanced NPC were enrolled and divided into two groups by the random number table method.TPF induction chemotherapy was applied to patients in both groups.The observation group received Aiyu Capsules combined with concurrent chemoradiotherapy,while the control group received concurrent chemoradiotherapy only.The efficacy,TCM syndrome score,adverse reactions,cellular immunity,and prognosis were compared between the two groups.After treatment,the patients were followed up to February 2022 and the survival rate difference between the two groups was compared.Results:A total of 130 patients completed the treatment,with 65 cases in each group.The disease control rate(DCR)in the observation group was higher than that in the control group(P<0.05),The proportion of radiotherapy interruption and the incidence of gradeⅢor above oral mucositis,leukopenia,severe anemia,nausea,and vomiting in the observation group were lower than those in the control group(P<0.05),and the synchronization completion rate was higher than that in the control group(P<0.05).Compared with the conditions before treatment,the CD3^(+)T cells,CD4^(+)T cells,CD4^(+)/CD8^(+),CD3-CD56^(+)NK cells,CD56brightNK cells,CD56dimNK cells,CD39^(+)CD73^(+)B cells,CD19^(+)B cells,Th17,Th17/Treg after treatment increased in both groups(P<0.05),while the Treg cells,CD8^(+)T cells and score of TCM syndrome decreased(P<0.05).After treatment,CD3^(+)T cells,CD4^(+)T cells,CD4^(+)/CD8^(+),CD3-CD56^(+)NK cells,CD56 brightNK cells,CD56 dimNK cells,CD39^(+)CD73^(+)B cells,CD19^(+)B cells,Th17,Th17/Treg in the observation group were higher than those in the control group(P<0.05),whil
