单孔机器人辅助前列腺癌根治术专家共识 被引量:2

Expert consensus statement of single-site robot-assisted radical prostatectomy from the Robotics Group of Urology branch of Chinese Medical Association

摘要 机器人辅助前列腺癌根治术(RARP)已成为治疗局限性前列腺癌(PCa)的主流术式。2008年以来,针对特定患者人群,国内外学者开始采用不同入路方式开展单孔RARP(ssRARP),取得了良好效果,并有较多文献发表。本共识编写组建议:ssRARP需要选择最佳适应证人群,特别是局限性、T1~T2c中低危PCa患者;根据患者不同特点、术者习惯和经验以及平台资源,具体制定腹膜外(含改良)、腹腔、会阴、da Vinci SP等不同入路方式ssRARP,并采取对应的泊机方式和手术步骤;需注重手术技巧改进和防治特异性并发症等。针对特定患者,个体化制定不同入路ssRARP和对应的操作,有利于在取得良好临床疗效基础上使患者获益更大。 Robot-assisted radical prostatectomy(RARP)has become the mainstream procedure for the treat⁃ment of localized prostate cancer(PCa).Since 2008,for specific patient populations,domestic and foreign scholars have begun to implement single-site robot assisted radical prostatectomy(ssRARP)with different approaches,which has achieved good results,and many literatures have been published.Based on the experience of each center,pub⁃lished articles,expert opinions,and questionnaires,to make recommendations for the surgical procedure names that have not been unified,and to conduct a consensus review of the existing ssRARP procedures and clinical data.Con⁃vened by the Robotics Group of Urology branch of Chinese Medical Association,the ssRARP consensus group sum⁃marized the consensus of experts on ssRARP through literature search and questionnaire survey.Current data suggest that ssRARP has a good effect.Different from conventional multiport RARP,the consensus group suggested that ss⁃RARP should be selected for the best indication population,especially for patients with localized,T1-T2C medium-low risk PCa;according to the different characteristics of the patients,the habits and experience of the surgeons and the platform resources,different approaches such as extraperitoneal(including modified),transperitoneal,transperi⁃neal,da Vinci SP and other approaches are specifically formulated for ssRARP,and the corresponding docking meth⁃ods and surgical procedures are adopted;focus on the improvement of surgical techniques and the prevention and treatment of specific complications.The individualized formulation of different ssRARP approaches and corresponding procedures will be more beneficial for specific patients on the basis of good clinical effect.
