摘要
目的探讨亚洲型A2FN顺行髓内钉治疗股骨干合并同侧股骨颈骨折的疗效。方法收集2015年4月-2020年4月收治的股骨干合并同侧股骨颈骨折患者28例病例资料,均采用亚洲型A2FN顺行髓内钉治疗,分析患者手术时间、术中出血量、骨折愈合时间、并发症发生情况以及临床疗效。结果28例均获得8~18个月随访,平均12个月,平均手术时间114.6 min(78~224 min),术中平均出血量150 mL(80~450 mL),平均住院时间15 d(11~24 d)。术后均无内固定失败、感染、股骨头坏死、深静脉血栓形成、肺栓塞及肢体严重畸形等并发症发生。所有患者均达到骨性愈合标准,股骨干平均愈合时间4.8个月(3~7个月),股骨颈平均愈合时间5.8个月(3~8个月)。按照Friedman-Wyman疗效评价标准:优22例,良5例,差1例,优良率为96.43%。结论亚洲型A2FN顺行髓内钉治疗股骨干合并同侧股骨颈骨折手术操作简单、微创,符合解剖和生物力学特点,短期疗效满意。
Objective To investigate the effect of Asian-type A2FN antegrade intramedullary nail on the treatment of ipsilateral femoral neck and shaft fractures.Methods A total of 28 patients with ipsilateral femoral neck and shaft fractures were collected from April 2015 to April 2020,all patients were treated with Asian-type A2FN antegrade intramedullary nail,and the operation time,intraoperative blood loss,fracture healing time,complications and clinical eficacy were observed.Results Twenty-eight patients were followed up,and the average follow-up time was 12 months(8~18 months),the average operation time was 114.6 min(78~224 min),the average intraoperative blood loss was 150 mL(80~450 mL),and the average hospital stay was 15 d(11~24 d).There were no complications such as failure of internal fixation,infection,femoral head necrosis,deep vein thrombosis,pulmonary embolism,and severe limb deformities.All patients reached the standard of bone healing,and the average healing time of the femoral shaft was 4.8 months(3~7 months),and the average healing time of the femoral neck was 5.8 months(3~8 months).According to Friedman-Wyman efficacy evaluation criteria:22 cases were excellent,5 cases were good,and 1 case was poor,the overall excellent and good rate was 96.43%.Conclusion The Asian-type A2FN antegrade intramedullary nail for the treatment of ipsilateral femoral neck and shaft fractures is simple,minimally invasive,in line with anatomical and biomechanical characteristics,and has a satisfactory short-term effect.
