摘要
丛集性头痛是最常见的三叉自主神经性头痛,表现为在丛集期内频繁出现的剧烈单侧头痛。疼痛部位多位于一侧眼眶或者额颞部,且大多数患者伴发头痛侧自主神经症状,如结膜充血、流泪、面部出汗、头面部水肿等。丛集性头痛具有较高的致残性,及时正确的诊断与治疗有助于缩短病程,减轻患者痛苦。近年来,国际上对丛集性头痛的诊治有了更多的循证医学证据。为进一步提高临床医师对丛集性头痛疾病的认识、管理与规范化诊疗,中华医学会神经病学分会头痛协作组的相关专家结合国内外临床实践,经过反复讨论后制订本指南。指南的内容包括丛集性头痛分类、诊断与鉴别诊断及治疗。
Cluster headache is the most common trigeminal autonomic cephalalgia,characterized by extremely painful,strictly unilateral and frequent headache attacks in orbital,supraorbital,and/or temporal regions during the cluster period.Most headache attacks are accompanied by ipsilateral cranial autonomic symptoms,such as conjunctival congestion,lacrimation,and facial sweating.Cluster headache has a high level of disability.Timely and accurate diagnosis and treatment contribute to a good prognosis for cluster headache.The past decade has witnessed more evidence-based medical data for the management of cluster headache.The purpose of this guideline is to improve the understanding,management,diagnosis and treatment of cluster headache.According to the cluster headache clinical advance and current situation in China,specialists of Headache Group of Chinese Society of Neurology drafted this guideline.This guideline includes classification,diagnosis and differential diagnosis,treatment of cluster headache.
