摘要
目的研究凉山州地区彝族和汉族盆腔炎性疾病(PID)的特点。方法选取100例妇科确诊的PID患者,其中彝族组60例,汉族组40例。收集患者的病历资料,观察比较两组患者基线资料、临床症状评分及中医证型分布情况。结果彝族组患者年龄、初婚年龄、高中及以上占比低于汉族组患者,孕次、产次、流产次数、不良生活史及盆腔手术史占比高于汉族组患者,差异均有统计学意义(P<0.05)。彝族组患者各项临床症状评分均高于汉族组患者,差异均有统计学意义(P<0.05)。彝族组患者肾虚血瘀型占比高于汉族组患者,气滞血瘀型占比低于汉族组患者,差异均有统计学意义(P<0.05)。结论彝族和汉族PID患者基本情况存在差异,可通过提高彝族女性文化程度,避免过早结婚过早性生活、避免非计划的频繁妊娠及流产,减少或避免不良生活习惯以降低PID的发生率。彝族患者临床症状较重,并且中医证型与汉族差别较大,采取临床针对性治疗能够提高治疗效果。
Objective To study the characteristics of pelvic inflammatory diseases(PID)of Yi and Han nationalities in Liangshan Prefecture.Methods 100 PID patients diagnosed by gynecology department were selected,including 60 patients of Yi nationality and 40 patients of Han nationality.The patients'medical record data were collected,the baseline data,clinical symptom scores and distribution of traditional Chinese medicine syndrome types between the two groups were observed and compared.Results The proportion of Yi nationality patients in age,age of first marriage,senior high school and above were lower than those of Han nationality patients,and the proportion of pregnancy,labor,abortion,adverse life history and pelvic surgery were higher than those of Han nationality patients,the differences were statistically significant(P<0.05).The scores of clinical symptoms in Yi nationality patients were higher than those of Han nationality patients,the diferences were statistically significant(P<0.05).The proportion of Yi nationality patients with kidney deficiency and blood stasis was higher than that of Han nationality patients,while the proportion of Qi stagnation and blood stasis was lower than that of Han nationality patients,the differences were statistically significant(P<0.05).Conclusion There are differences in the basic conditions of the patients with PID between the Yi nationality and the Han nationality.The incidence of PID can be reduced by improving the educational level of Yi women,avoiding premarital marriage and sexual life,avoiding unplanned frequent pregnancy and abortion,and reducing or avoiding bad living habits.Yi nationality patients have severe clinical symptoms,and their TCM syndrome types are quite different from those of Han nationality patients.They should be treated in a targeted way to improve the therapeutic effect.
