摘要
总结钦丹萍教授内治与外治结合治疗炎症性肠病的经验。临证强调辨寒热,重视寒热错杂之病机;辨脏腑,重视肝脾、肺脾之间的关系,善用宣肺利水、调肝理脾、温阳散寒之法。针对难治型炎症性肠病,善用收敛生肌之品,强调活血化瘀之法,常用雷公藤或雷公藤成分提取物以发挥免疫调控之效;对活动期溃疡性结肠炎,倡导中药灌肠以快速缓解炎症活动,并建立中药灌肠治疗二步法,即先以中药灌肠诱导炎症缓解,继以灌肠为基础,酌情联合他法以维持疗效。
This paper summarized the experience of Professor QIN Danping in treating inflammatory bowel disease by internal and external treatment with traditional Chinese medicine(TCM).Professor QIN emphasizes the differentiation of cold and heat and pays attention to the existence of mixed cold and heat conditions.The differentiation of zang-fu organs(脏腑)is critical as well,and the relationship between liver and spleen,and lung and spleen should be clarified.He usually carries out clinical treatment with the methods of disfussing the lung and promoting urination,regulating liver and spleen,and warming yang and dispelling cold.For refractory inflammatory bowel disease,astringent products with the function of closing sore and engendering flesh are used,and method of promoting blood circulation and removing blood stasis is emphasized.Leigongteng(Radix Tripterygii Wilfordii)and its extracts could be used to regulate the immune system.For active ulcerative colitis,a two-step method of TCM enema therapy for rapid relief of inflammation has been established,that is,first,simple TCM enema to induce inflammation relief,and then,other methods in combination as appropriate to maintain the stability of remission based on enema.
