年龄因素对右美托咪定滴鼻用于TTE检查患儿镇静药效学的影响

Effect of age factors on pharmacodynamics of intranasal dexmedetomidine for sedation in pediatric patients undergoing transthoracic echocardiography

摘要 目的评价年龄因素对右美托咪定滴鼻用于经胸心脏超声(TTE)检查患儿镇静药效学的影响。方法选取本院2019年8月至2022年5月行TTE检查的患儿,年龄1~24月,ASA分级Ⅰ~Ⅲ级。试验Ⅰ:根据患儿年龄分为4组:1~6月龄组、7~12月龄组、13~18月龄组和19~24月龄组。右美托咪定滴鼻起始剂量为2.0μg/kg,剂量梯度为0.1μg/kg,采用序贯法进行试验,采用Dixon-Massey法计算右美托咪定镇静的ED50及95%可信区间。试验Ⅱ:100例患儿按年龄分为4组(n=25):1~6月龄组、7~12月龄组、13~18月龄组和19~24月龄组。再按右美托咪定滴鼻剂量随机分为5个亚组(n=5):2.1μg/kg亚组、2.2μg/kg亚组、2.3μg/kg亚组、2.4μg/kg亚组和2.5μg/kg亚组。合并试验Ⅰ和Ⅱ,采用probit法计算右美托咪定镇静的ED95及95%可信区间。结果共纳入220例患儿。各组右美托咪定镇静的ED50和ED95比较差异无统计学意义(P>0.05)。结论对于1~24月龄患儿,右美托咪定滴鼻用于TTE检查镇静的药效学无明显年龄差异。 Objective To evaluate the effect of age factors on the pharmacodynamics of intranasal dexmedetomidine for sedation in the pediatric patients undergoing transthoracic echocardiography(TTE).Methods American Society of Anesthesiologists Physical Status classificationⅠ-Ⅲpediatric patients,aged 1-24 months,undergoing TTE from August 2019 to May 2022,were selected.This trial was performed in two parts.PartⅠPediatric patients were divided into 4 age groups:1-6 month group,7-12 month group,13-18 month group and 19-24 month group.The initial dose of dexmedetomidine was 2.0μg/kg in 0.1μg/kg increment/decrement.The dose of dexmedetomidine was determined by using modified Dixon′s up-and-down method.The ED50 and 95%confidence interval of intranasally administered dexmedetomidine for sedation were calculated by the Dexon-Massey method.PartⅡOne hundred patients were divided into 4 age groups(n=25 each):1-6 month group,7-12 month group,13-18 month group and 19-24 month group.The 4 groups were further divided into 5 subgroups(n=5 each)according to the dose of dexmedetomidine:2.1μg/kg subgroup,2.2μg/kg subgroup,2.3μg/kg subgroup,2.4μg/kg subgroup,and 2.5μg/kg subgroup.PartⅠand partⅡtrials were combined,and the ED95 and 95%confidence interval of intranasally administered dexmedetomidine for sedation were calculated using the probit method.Results A total of 220 pediatric patients were enrolled.There was no significant difference in ED50 and ED95 of dexmedetomidine intranasally administered for sedation among groups(P>0.05).Conclusions The pharmacodynamics of intranasal dexmedetomidine for sedation shows no significant difference in age in the pediatric patients aged 1-24 months undergoing TTE.
