系统免疫炎症指数与冠心病患者冠状动脉病变程度的相关性研究 被引量:9

Correlation between systemic immune-inflammation index and the degree of coronary artery stenosis in patients with coronary heart disease

摘要 目的探讨系统免疫炎症指数(SII)与冠心病(CHD)患者冠状动脉病变程度的相关性。方法回顾性横断面研究。连续纳入2008年1月至2017年12月期间新疆医科大学第一附属医院心脏中心收治的6046例CHD患者作为研究对象,收集临床资料及冠状动脉血管成像结果,Gensini评分定量评估冠状动脉狭窄程度。根据Gensini评分三分位数分为:低值组(<11分,2011例)、中值组(11~38分,2018例)和高值组(>38分,2017例)。采用Pearson相关性分析和多元线性回归分析评估SII与Gensini评分的相关性。结果三组间的性别、心率、高血压、白细胞计数、淋巴细胞计数、血小板计数和血肌酐的差异均有统计学意义(均为P<0.05)。Pearson相关分析表明,CHD患者的SII与Gensini评分呈正相关(r=0.115,P<0.05)。亚组分析结果显示,在性别、是否合并糖尿病、是否合并高血压中SII与Gensini评分呈正相关,差异均有统计学意义(r分别为0.135、0.138、0.139、0.123、0.155、0.122,均为P<0.05)。女性患者的相关性比男性患者的高,非糖尿病患者的相关性比糖尿病患者的高,非高血压患者的相关性比高血压患者的高(0.138比0.135,0.139比0.123,0.155比0.122)。多元线性回归分析显示,心率、白细胞计数、淋巴细胞计数、血肌酐和SII均与Gensini评分具有独立的相关关系,且白细胞计数对Gensini评分的影响最大(r=0.095,P<0.001);在亚组人群中,SII与Gensini评分依然具有独立的相关关系(P<0.05)。结论控制性别等混杂因素后,CHD患者的SII与Gensini评分呈显著独立正相关。SII或许可以用于CHD患者的病情评估。 Objective To explore the correlation between systemic immune-inflammation index(SII)and the degree of coronary artery stenosis in patients with coronary heart disease(CHD).Methods This was a retrospective cross-sectional study.The clinical data and coronary angiography results of 6046 CHD patients admitted to the Department of Cardiology of the First Affiliated Hospital of Xinjiang Medical University from January 2008 to December 2017,the degree of coronary artery stenosis was quantitatively evaluated by Gensini score.Patients were divided into three groups according to the tertile of Gensini score:low value group(<11 points,2011 cases),median value group(11-38 points,2018 cases)and high value group(>38 points,2017 cases).Pearson correlation analysis and multiple linear regression analysis were used to evaluate the correlation between SII and Gensini score.Results The differences in gender,heart rate,hypertension,white blood cell count(WBC),lymphocyte count(Lym),platelet count(Plt)and serum creatinine(Scr)among the three groups were statistically significant(all P<0.05).Pearson correlation analysis showed a positive correlation between SII and Gensini score in patients with CHD(r=0.115,P<0.05).Subgroup analysis showed that SII was positively correlated with Gensini score in gender,presence of comorbid diabetes,and presence of comorbid hypertension,all with statistically significant differences(r=0.135,0.138,0.139,0.123,0.155,0.122,respectively,all P<0.05).The correlations were more significant in female patients than in male patients,in non-diabetic patients than in diabetic patients,and in non-hypertensive patients than in hypertensive patients(0.138 vs.0.135,0.139 vs.0.123,0.155 vs.0.122).Multiple linear regression analysis showed that heart rate,WBC,Lym,Scr and SII were independently correlated with Gensini score and that WBC had the greatest effect on Gensini score(standardized regression coefficient=0.095,P<0.001).The SII remained independently correlated with Gensini score in the subgroup population.Conclus
