摘要
目的 对不同类型先天性耳廓畸形患儿行耳模矫正后疗效的差异进行比较分析。方法 以38例(76耳,均为双侧)先天性耳廓畸形患儿为研究对象,根据畸形类型分三组:杯状耳为1组(杯状耳组),耳甲腔异常凸起为2组(耳甲腔组),其它畸形为3组(其它畸形组)。患儿均进行耳模矫正,治疗结束后随访3个月。比较不同畸形的即时成功率、最终矫正成功率及复发率。结果 治疗结束时,三组畸形即时成功率差异无统计学意义(P=0.447,P>0.05)。随访3个月后,杯状耳组最终矫正成功率(11.11%)低于耳甲腔组(81.25%)、其它畸形组(100.00%)(P=0.006,P=0.000),且耳甲腔组低于其它畸形组(P=0.015),差异有统计学意义(均P<0.05)。杯状耳组复发率(100.00%)高于耳甲腔组(43.75%)、其它畸形组(2.74%)(P=0.024,P=0.000),且耳甲腔组高于其它畸形组(P=0.000),差异有统计学意义(均P<0.05)。结论 不同畸形行耳模矫正后均可取得较好的即时疗效,但最终疗效存在差异。杯状耳、耳甲腔异常凸起较其它畸形最终矫正成功率低、复发率高,其中杯状耳矫正难度最大、复发率最高。
Objective To report outcomes of ear molding in young children with different types of congenital auricle deformities.Methods Based on the type of deformities,38 cases(76 ears)of congenital auricle deformities were grouped into cup ear(group 1),conchal crus(group 2),and other deformities(group 3).All cases were treated by ear molding and followed up for 3 months after treatment.The rates of immediate success,endpoint success and recurrence were compared among deformity groups.Results The rate of immediate success was not significantly different among the three groups(P=0.447,P>0.05).At 3 months follow-up,the rate of success was lower in group 1(11.11%)than in groups 2(81.25%)group 3(100.00%)(P=0.006,P=0.000),and lower in group 2 than in group 3(P=0.015).The rate of recurrence in group 1(100.00%)was higher than both group 2(43.75%)and 3(2.74%)(P=0.024,P=0.000),and higher in group 2 than in group 3(P=0.000).Conclusion While ear molding may yield good immediate correction in all types of auricular deformities,the long term outcome differs among different types of deformities with lower success rate and higher recurrence rate in cup ear and conchal crus as compared to other deformities.Ear molding is the least effective in cup ear with the highest recurrence rate.
