摘要
目的探讨肺炎支原体抗原(mycoplasma pneumonia antigen,MP Ag)联合肺炎支原体免疫球蛋白M抗体(mycoplasma pneumoniae immunoglobulin M antibody,MP-IgM)诊断肺炎支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)的价值。方法选取2019年1月至2019年10月攀枝花学院附属医院收治的108例高危MPP患者,所有患者均采用抗原量子点标记技术检测MP Ag,采用胶体金法检测MP-IgM水平,咽拭子标本进行肺炎支原体(mycoplasma pneumoniae,MP)培养和鉴定。统计分析MP Ag、MP-IgM对MPP的诊断价值,并分析MP Ag联合MP-IgM方法诊断MPP的效能。结果108例高危MPP对象中经MP培养法确诊为MPP患者共45例,余63例未患MPP。45例确诊为MPP的患者中经MP Ag、MP-IgM及MP Ag+MP-IgM诊断阳性者分别为40例、43例、45例;63例未患MPP者中经MP Ag、MP-IgM及MP Ag+MP-IgM诊断阳性者分别为3例、4例、3例。MP Ag+MP-IgM诊断MPP的灵敏度、准确度、约登指数均高于MP Ag、MP-IgM单独诊断,特异度高于MP-IgM单独诊断,但与MP Ag单独诊断无差异。结论MP Ag联合MPP IgM诊断MPP的效能较高,与MP培养法检测结果的一致性也较高,具有一定的临床推广与应用价值。
Objective To explore the value of mycoplasma pneumoniae antigen(MP Ag)combined with mycoplasma pneumoniae immunoglobulin M antibody(MP IgM)in the diagnosis of mycoplasma pneumoniae pneumonia(MPP).Methods Total of 108 high-risk MPP patients admitted to the Affiliated Hospital of Panzhihua University from January 2019 to October 2019 were selected.All patients were tested for MP Ag by antigen quantum dot labeling technology,and the level of MP-IgM was detected by colloidal gold method.Pharyngeal swab samples were cultured and identified for mycoplasma pneumoniae(MP).The diagnostic value of MP Ag and MP IgM for MPP,as well as the efficacy of MP Ag combined with MP IgM method in diagnosing MPP were analyzed.Results Among the 108 high-risk MPP patients,total of 45 cases were diagnosed with MPP through MP culture,while the remaining 63 did not suffer from MPP.Among the 45 patients diagnosed with MPP,there were 40 cases of positive diagnosis for MP Ag,43 cases of positive diagnosis for MP IgM,and 45 cases of positive diagnosis for MP Ag+MP IgM respectively.Among the 63 patients without MPP,3 cases were diagnosed positive for MP Ag,4 cases for MP IgM,and 3 cases for MP Ag+MP IgM respectively.The sensitivity,accuracy,and Yoden index of MP Ag+MP IgM in the diagnosis of MPP were higher than those of MP Ag and MP IgM alone,and the specificity was also higher than that of MP IgM alone,but there is no significant difference with MP Ag alone.Conclusion The combination of MP Ag and MPP IgM has a higher efficacy in diagnosing MPP,and the consistency with the results detected by MP culture method is also high.The combination diagnosis has certain clinical promotion and application value.
