摘要 目的分析HPV52和HPV58感染与患者发生宫颈上皮病变风险的相关性。方法选取2018年10月至2021年10月进行检查的816例人乳头瘤病毒(HPV)感染者,患者分别接受宫颈液基薄层细胞学(TCT)检查、HPV基因分型检测、阴道微生态检查。将感染高危型HPV且TCT结果为宫颈上皮病变的294例患者分为低级别鳞状上皮病变组(LSIL)138例、高级别鳞状上皮病变组(HSIL)127例、宫颈癌组29例。分析不同宫颈上皮病变程度患者感染高危型HPV的型别分布情况,评估HPV52和HPV58感染、阴道微生态异常与宫颈上皮病变程度的相关性。结果816例HPV感染者中检出高危型HPV感染者294例,其中感染率较高的型别是HPV16、52、58,感染率分别为36.73%、15.99%、13.95%。LSIL组、HSIL组、宫颈癌组患者的HPV52和HPV58感染率以及pH值、白细胞酯酶、唾液酸苷酶、过氧化氢(H_(2)O_(2))阳性率随着宫颈上皮病变程度加重而依次递升(P<0.05)。结论HPV16、52、58属于高危型HPV感染型别,其中HPV52和HPV58感染、阴道微生态异常与宫颈上皮病变程度呈正相关,感染率和阳性率随着宫颈上皮病变程度加重而升高。 Objective To analyze the correlation between HPV52 and HPV58 infections with the risk of cervical epithelial lesions.Methods A total of 816 patients with human papillomavirus(HPV)infection examined in our hospital from October 2018 to October 2021 were selected.All patients received cervical liquid-based thin-layer cytology(TCT)examination,HPV genotyping test and vaginal microecological examination.A total of 294 patients with high-risk HPV infection and cervical epithelial lesions were allocated to low-grade squamous lesions group(LSIL group,n=138),high-grade squamous lesions group(HSIL group,n=127),and cervical cancer group(n=29).The distribution of high-risk HPV types in patients with different degrees of cervical epithelial lesions was analyzed,and the correlation between HPV52 and HPV58 infections,vaginal micro-ecological abnormalities with the degree of cervical epithelial lesions was evaluated.Results Among these patients,294/816 patients were infected with high-risk HPV.HPV16,52,and 58 were the top three subtype of HPV infections,with the infection rate of 36.73%,15.99%,and 13.95%,respectively.The infection rates of HPV52 and HPV58,pH level,as well as the positive rates of leukocyte esterase,sialidase,and hydrogen peroxide(H_(2)O_(2))in the LSIL group,HSIL group,and cervical cancer group increased sequentially with the severity of cervical epithelial lesions(P<0.05).Conclusion HPV16,52,and 58 are high-risk types of HPV infections.Among them,HPV52 and HPV58 infections,and vaginal microecological abnormalities were positively correlated with the degree of cervical epithelial lesions.The infection rates of HPV52 and HPV58 and their positive rates increased with the aggravation of cervical epithelial lesions.
