摘要
目的探讨T2DM患者内脏脂肪面积(VFA)与糖尿病周围神经病变(DPN)的相关性。方法选取2020年8月至2022年4月于临沂市人民医院内分泌代谢病科住院治疗的T2DM患者863例,分为单纯T2DM组(n=497)和T2DM合并DPN组(DPN,n=366),比较两组一般资料及生化指标。Spearman相关分析DPN与各指标的相关性,Logistic回归分析DPN的影响因素。结果与T2DM组比较,DPN组年龄、DM病程、FPG、HbAlc、UACR升高,FC-P、BMI、VFA、皮下脂肪面积(SFA)、血清游离三碘甲状腺原氨酸(FT_(3))、SUA、谷草转氨酶、谷丙转氨酶降低(P<0.05)。Spearman相关分析显示,DPN与年龄、DM病程、FPG、HbA1c、UACR呈正相关(P<0.05),与BMI、FC-P、VFA、SFA、FT3呈负相关(P<0.05)。Logistic回归分析显示,DM病程、FPG、HbA1c、UACR及VFA是DPN的影响因素。结论低水平VFA是DPN的危险因素。
Objective To explore the correlation between visceral fat area(VFA)and diabetic peripheral neuropathy(DPN)in patients with type 2 diabetes mellitus(T2DM).Methods 863 T2DM patients hospitalized in Linyi People's Hospital from August 2020 to April 2022 were selected and divided into simple T2DM group(n=497)and T2DM combined with DPN group(DPN,n=366).The general data and biochemical indexes of the two groups were compared.Spearman correlation analysis was used to analyze the correlation between DPN and each index.Logistic regression analysis was used to analyze the influencing factors of DPN.Results Compared with T2DM group,age,DM duration,FPG,HbAic and UACR were increased,and FC-P,BMI,VFA,subcutaneous fat area(SFA),free triodothyronine,SUA,aspartate aminotransferase and alanine aminotransferase were decreased in DPN group(P<0.05).Spearman correlation analysis showed that DPN was positively correlated with age,DM duration,FPG,HbAic,UACR(P<0.05),and negatively correlated with BMI,FC-P,VFA,SFA,and FT,(P<0.05).Logistic regression analysis showed that DM duration,FPG,HbAic,UACR and VFA were influence factors of DPN.Conclusion Low VFA is a risk factor for the development of DPN.
