摘要
目的:探讨绝经后2型糖尿病(T2DM)女性OPG基因rs6993813、rs4355801位点基因多态性及突变与骨量异常的关系,为绝经后T2DM伴骨质疏松(OP)人群提供新的临床预测指标及诊疗依据。方法:选取石河子大学医学院第一附属医院于2020年10月至2021年10月期间就诊的200例绝经后女性作为研究对象,根据研究对象的糖耐量试验及BMD测量结果分为四组:血糖正常伴骨量正常组(n=52)、T2DM伴骨量正常组(n=47)、血糖正常伴骨量异常组(n=43)及T2DM伴骨量异常组(n=58);记录患者绝经年限等一般基线资料,检测其代谢性相关生化指标,并测定OPG基因rs6993813和rs4355801位点的基因分型。结果:OPG基因rs6993813位点:在D组中,CC/TC基因型(突变型)的P水平较TT基因型(野生型)偏低(P<0.05),而CC/TC基因型(突变型)的BMD(NOF)水平较TT基因型(野生型)偏高(P<0.05)。OPG基因rs4355801位点:在B组中,GG/AG基因型(突变型)的FPG、HbA1c水平较AA基因型(野生型)显著偏低(均P<0.05)。在D组中,GG/AG基因型(突变型)的BMD(L1-4)水平较AA基因型(野生型)显著偏高(P<0.05)。多元线性回归分析显示:甘油三酯的降低以及rs4355801位点突变的减少是BMD(L1-4)降低的危险因素;绝经年限的增加以及高密度脂蛋白的降低是BMD(NOF)降低的危险因素。结论:绝经后T2DM女性OPG基因rs6993813、rs4355801位点基因多态性可能与骨代谢无关;但其rs6993813位点基因突变可能与骨代谢相关;其rs4355801位点基因突变可能与骨代谢、糖代谢相关;长绝经年限、低TG、低HDL-C可能是BMD降低的危险因素。
Objective:To investigate the relationship between gene polymorphisms and mutations at the rs6993813 and rs4355801 loci of the OPG gene and bone mass abnormalities in postmenopausal women with type 2 diabetes mellitus,and to provide a new clinical predictor and basis for the treatment of postmenopausal T2DM with osteoporosis.Methods:Two hundred postmenopausal women attending the First Affiliated Hospital of Shihezi University School of Medicine between October 2020 and October 2021 were selected for the study and were divided into four groups according to their glucose tolerance test and BMD measurements:normal glucose with normal bone mass(n=52),T2DM with normal bone mass(n=47),normal glucose with abnormal bone mass(n=43)and T2DM with abnormal bone mass(n=58);general baseline information such as the patient's age at menopause was recorded;their metabolism-related biochemical parameters were measured;and the genotyping of the rs6993813 and rs4355801 loci of the OPG gene was determined.Results:OPG gene rs6993813 locus:In group D,P levels were lower in CC/TC genotype(mutant)compared with TT genotype(wild type)(P<0.05),while BMD(NOF)levels were higher in CC/TC genotype(mutant)compared with TT genotype(wild type)(P<0.05).rs4355801 locus of OPG gene:In group B,the FPG and HbA1c levels were significantly lower in the GG/AG genotype(mutant)compared to the AA genotype(wild type)(all P<0.05).In group D,BMD(L1-4)levels were significantly higher in the GG/AG genotype(mutant)compared to the AA genotype(wild type)(P<0.05).Multiple linear regression analysis shows:decreased TG and decreased mutations at the rs4355801 locus were risk factors for decreased BMD(L1-4);increased age at menopause and decreased HDL-C were risk factors for decreased BMD(NOF).Conclusion:Gene polymorphisms at the rs6993813 and rs4355801 loci of the OPG gene in postmenopausal women with T2DM may not be associated with bone metabolism;however,the gene mutation at its rs6993813 locus may be related to bone metabolism;and the gene mutation at its rs4355801 l
