摘要
总结8例新生儿腹裂行一期修补术的多学科协作护理体会。针对该组患儿出生后需即刻手术,肠管长时间暴露于体外易导致水、电解质紊乱、膨出物破损感染、低体温等护理难点,建立多学科协作团队,实现全程管理;融合加速康复理念,缩短术前准备时间;术前保护肠管膨出物,维持体温恒定,加强体液管理;术后加强监测,避免腹腔间隙综合征、营养不良等并发症;开展阶段式健康教育及随访,促进患儿术后快速康复,提高患儿生活质量。经过积极治疗与精心护理,该组患儿均在出生后3 h内完善术前准备,顺利完成手术,术后恢复良好,好转出院。
To summarize the experience of multidisciplinary cooperative nursing in 8 cases of neonatal gastroschisis undergoing one-stage repair.In view of the children in this group who needed immediate surgery after birth,and the exposure of intestine in vitro for a long time could easily lead to water and electrolyte disorders,swelling damage and infection,and hypothermia and other difficulties,a multidisciplinary collaborative team was established to achieve whole-process management.Combined with accelerated rehabilitation concept,we shortened preoperative preparation time,protected intestinal extrusions before the surgery to maintain constant body temperature and strengthen body fluid management,strengthened postoperative monitoring to avoid abdominal space syndrome,malnutrition and other complications.Staged health education and follow-up were carried out to promote their rapid recovery after surgery and improve the quality of life of children.After active treatment and careful nursing,all the children in this group completed the preoperative preparation within 3 hours after birth,the operationwas successful,and the postoperative recovery was good.They recovered and were discharged.
