摘要
目的分析密度梯度离心法对行夫精人工授精(AIH)患者精子DNA碎片率(DFI)的改善作用,以及精子DFI与AIH妊娠率的相关性。方法选取2019年9月至2021年10月在内蒙古自治区妇幼保健院实施AIH治疗患者的320个周期作为研究对象,采用流式细胞法获得密度梯度离心法优选前后的精子DFI,并进行统计学分析。结果AIH周期患者精子DFI异常(≥15%)率为25.94%;精子优选处理后DFI得到了显著改善(P<0.01),经密度梯度离心法处理后DFI≥15%的精液标本占比从25.94%下降到了9.07%,经随访95.24%的妊娠个案来自优选处理后精子DFI<15%的分组。结论密度梯度离心法对精子的DFI有显著改善作用;AIH周期的男性患者处理前精子DFI与该周期妊娠率无显著相关性;优选处理后较低精子DFI对应较高的妊娠率,优选处理后的精子DFI对患者的妊娠结局更有预测价值。
Objective To analyze the improvement effect of density gadient centifugation on sperm DNA fmagmentation index(DFI)of artificial insemination by husband(AIH)patients,and the correlation between spem DFI and AIH pregnancy mate.Methods From September 2019 to October 2021,320 cyeles of patients receiving AIH treatment in Inner Mongolia Matemal and Child Health Hospital were selected as research objects.Flow cytometry was used to obtain spemm DFT before and after optimization by density gradient centrifugation method,and statistical analysis was conducted.Results The abnormal sperm DFI(≥15%)rate of AIH cyecle patients was 25.94%.Sperm DFI was significantly improved after optimal sperm treatment(P<0.01),the proportion of semen samples with DFI≥15%decreased from 25.94%to 9.07%after density gradient centrifuge treatment,and after fllow-up,95.24%of pregnancy cases were from the group with DFI<15%after optimal sperm teatment.Conclusions Density gradient centrifugation can significantly improve sperm DFI.There was no significant correlation between pre-treatment spem DFT and pregnancy rate in men with AIH cycle.The lower spermn DFI after optimal treatment corresponds to higher pregnancy rate,and spem DFI after optimal treatment is more valuable to predict the pregnancy outcome of patients.
