摘要
“胆主决断”是源自于《黄帝内经》的重要理论之一,在中医藏象学论述中,胆既为六腑亦是奇恒之腑,其不仅是解剖学概念中的一个独立器官,同时是代表诸多特定生理功能的概念总称,其中“主决断”是胆最重要的生理特性之一。然古今对“胆主决断”的功能阐释大多集中在对人体精神情志的调节控制上,往往淡化其在消化方面的重要功能作用。该文从理论研究和功能作用两部分进行探究,发现“胆主决断”的本质主要源于胆具有中正的特性,从经络归属、解剖位置、脏腑归属和生理特性等角度对胆的中正特性展开全面解读,有助于对其功能作用的深层把握。“胆主决断”的功能主要体现在“决断”情志和“决断”消化两个方面,其中胆汁与胆气是二者共同的生理基础。“决断”情志失常临床常见郁证、痴呆等疾病,“决断”消化失常所见疾病较少,以纳呆、厌油腻等临床症状为主,方药多用温胆汤、小柴胡汤和龙胆泻肝汤等方辨证施治。该文通过系统论述,以期提供理论探讨,加强该理论的相关研究以更好作用于临床。
“Gallbladder controlling decision”is one of the important theories derived from Huangdi's Internal Classic.In traditional Chinese medicine Zangxiang theory,the gallbladder is not only an independent organ in the anatomical concept,but also a general term representing many specific physiological functions.Among them,“controlling decision”is one of the most important physiological characteristics of the gallbladder.However,the ancient and modern functional interpretation of“gallbladder controlling decision”mostly focuses on the regulation and control of human spirit and emotion,often weakening its important function in digestion.This paper explored from theoretical research and functional function.It is found that the essence of“gallbladder controlling decision”mainly stems from the positive characteristics of gallbladder.A comprehensive interpretation of the positive characteristics of gallbladder from the perspectives of meridian attribution,anatomical location,viscera attribution and physiological characteristics is helpful to deeply grasp its functional function.The function of“gallbladder controlling decision”is mainly reflected in“emotion decision”and“digestion decision”,in which bile and gallbladder Qi are the common physiological basis.“Emotion decision”disorder is common in clinic,such as depression syndrome and dementia.There are few diseases seen in“digestion decision”disorder.It is mainly characterized by clinical symptoms such as stupidity and greasiness.The prescriptions are mostly treated by syndrome differentiation,such as Wendan Decoction(温胆汤),Xiaochaihu Decoction(小柴胡汤)and Longdan Xiegan Decoction(龙胆泻肝汤).Through systematic discussion,this paper hopes to provide the theoretical discussion and strengthen the relevant research of this theory in order to better play a role in clinic.
