摘要
胃癌前病变(precancerous lesion of gastric cancer, PLGC)是临床疑难疾病,现代医学缺乏特异性的治疗方法。谢晶日教授行医执教多年,临证经验颇丰,特别是运用祖国医学治疗PLGC见解独到。作者介绍了谢晶日教授对于PLGC的辨治思路与临证经验。谢师认为,PLGC多表现为本虚标实之象,病机可概括为“虚”“瘀”“毒”三个方面,“虚”指气虚、阴虚,“瘀”指气郁、血瘀,“毒”指湿毒、热毒。治疗上,主张四诊互参,防治兼顾,以顾护脾胃为先,并贯穿治疗始末,以化瘀毒为标,逐秽浊之壅塞通达脉络,同时揆病证之变化随证加减。此外,谢师还特别强调中西相辅,集各家之长,注重未病先防。
Precancerous lesions of gastric cancer(PLGC)is a clinically difficult disease,and modern medicine lacks specific treatment methods.Professor XIE Jingri has practiced medicine and taught for many years,and has a wealth of clinical experience,especially his unique insights into the treatment of PLGC with Chinese medicine.The author introduced Professor XIE Jingri's syndrome differentiation and treatment of PLGC and his clinical experience.Professor XIE believes that PLGC mostly manifests itself as a sign of deficiency and superficiality,and its pathogenesis can be summarized as"deficiency","stasis"and"toxin"."Deficiency"refers to Qi deficiency and Yin deficiency,"stasis"refers to Qi depression and blood stasis and"toxin"refers to damp toxin and heat toxin.In terms of treatment,it is advisable to take care of spleen and stomach as the first and throughout the treatment and removing blood stasis and toxin is the second method to eliminate the obstruction of turbidity and dredge the veins.At the same time,the changes of the disease syndromes are added and subtracted with the syndrome.In addition,Professor XIE also particularly emphasized the combination of traditional Chinese medicine and Western medicine,combining the strengths of each expert and focusing on prevention before illness.
