摘要
目的研究循证护理对输尿管结石患者钬激光碎石术后并发症和应激反应的影响。方法选取2020年1月-2021年6月我院接受钬激光碎石术治疗的输尿管结石患者154例,随机分为研究组和对照组,每组77例。对照组采用常规护理方案,研究组采用循证护理方案,比较两组术后并发症发生情况、应激反应[皮质醇(Cor)、丙二醛(MDA)、超氧化物歧化酶(SOD)]、疼痛程度、心理状态及术后恢复情况。结果研究组术后并发症总发生率低于对照组(P<0.05);研究组术后24 h Cor、MDA低于对照组,SOD高于对照组(P<0.05);研究组术后24、48 h VAS评分低于对照组(P<0.05);研究组护理后抑郁评分和焦虑评分低于对照组(P<0.05);研究组开始进食时间、肛门排气时间、下床活动时间、住院时间短于对照组(P<0.05)。结论循证护理能够有效减少输尿管结石患者钬激光碎石术后并发症,降低应激反应水平,缓解疼痛,改善不良心理状态,促进术后恢复。
Objective To study the effect of evidence-based nursing on postoperative complications and stress response in patients with ureteral calculi after holmium laser lithotripsy.Methods A total of 154 patients with ureteral calculi who underwent holmium laser lithotripsy in our hospital from January 2020 to June 2021 were randomly divided into study group and control group,with 77 patients in each group.The control group was given routine nursing program,and the study group was given evidence-based nursing program.The incidence of postoperative complications,stress response[cortisol(Cor),malondialdehyde(MDA),superoxide dismutase(SOD)],pain degree,psychological state and postoperative recovery were compared between the two groups.Results The total incidence of postoperative complications in the study group was lower than that in the control group(P<0.05).At 24 h after operation,Cor and MDA in the study group were lower than those in the control group,and SOD was higher than that in the control group(P<0.05).The VAS scores of the study group at 24 and 48 h after operation were lower than those of the control group(P<0.05).The scores of depression and anxiety in the study group were lower than those in the control group(P<0.05).The time of starting eating,anal exhaust,getting out of bed and hospitalization in the study group were shorter than those in the control group(P<0.05).Conclusion Evidence-based nursing can effectively reduce the complications after holmium laser lithotripsy in patients with ureteral calculi,reduce the level of stress response,relieve pain,improve adverse psychological state,and promote postoperative recovery.
