摘要
目的对比观察单次平面回波成像(SS-EPI)和分段读出平面回波成像(RS-EPI)不同b值的图像质量和显示直肠癌病灶的效果,选择最优的b值。方法对48例直肠腺癌患者行b值1000 s/mm^(2)的SS-EPI和多个b值的RS-EPI,对比图像质量和病变信号强度分级,测量并比较不同b值下肿瘤信噪比(SNR)、对比噪声比(CNR)、信号强度比(SIR)和表观扩散系数(ADC)。结果S_(1000)、R_(1000)、R_(2000)、R_(3000)图像质量评分差异均有统计学意义(P<0.001),且R_(1000)的图像质量评分最高,R_(3000)的图像质量评分最低。R_(1000)、R_(2000)、R_(3000)图像中Ⅰ型数量逐渐减少,Ⅱ型数量逐渐增多,Ⅲ型仅存在于R_(2000)、R_(3000)中;S_(1000)、R_(1000)、R_(2000)、R_(3000)的SNR、CNR差异均具有统计学意义。S_(1000)与R_(1000)、R_(2000)的SIR差异无统计学意义,RS-EPI其余各b值SIR差异均具有统计学意义。S_(1000)与R_(1000)ADC值差异无统计学意义,其余各b值两种序列差异均具有统计学意义。结论b值为1000、2000、3000 s/mm^(2)的DWI图像中,b=1000 s/mm^(2)时,RS-EPI的图像质量最优,且b值同样为1000 s/mm^(2)时,RS-EPI显示直肠癌病灶优于SS-EPI。
Objective To compare the image quality of SS-EPI and RS-EPI with different b value and the effect of displaying rectal cancer lesions,and to select the best b value in the clinical DWI test.Methods 48 patients with rectal adenocarcinoma underwent single-shot echo-planar imaging(SS-EPI)with a b value of 1000 s/mm^(2)and segmented read-out echo-planar imaging(RS-EPI)with multiple b values.The image quality and lesion signal were compared.Intensity grading,measuring and comparing tumor signal-to-noise ratio(SNR),contrast-to-noise ratio(CNR),signal-intensity ratio(SIR)and apparent diffusion coefficient(ADC)at different b values.Results The image quality scores of S_(1000),R_(1000),R_(2000),and R_(3000)were significantly different(P<0.001),and the image quality score of R_(1000)was the highest,and the image quality score of R_(3000)was the lowest.In R_(1000),R_(2000),and R_(3000)images,the number of typeⅠgradually decreased,the number of typeⅡgradually increased,and typeⅢonly existed in R_(2000)and R_(3000);when the b value was 1000,the SNR,CNR,and SIR of RS-EPI were higher than those of SS-EPI.With comparing SNR,CNR,S_(1000)、R_(1000)、R_(2000)、R_(3000)has statistically significance.There was no statistic difference between S_(1000)and R_(1000)、R_(2000)in SIR.And difference among all b values for ADC were statistic different,except for S_(1000)and R_(1000).Conclusion In DWI images with b values of 1000,2000 and 3000 s/mm^(2),when b=1000 s/mm^(2),the image quality of RS-EPI is the best,and when the b value is also 1000 s/mm^(2),RS-EPI shows rectal cancer lesions with excellent on SS-EPI.
