摘要
基于整体观念和中医经典理论,从脏腑、气血、营卫、壮火阴火、升降、燥湿、痰瘀、正虚等不同维度探讨脱发的中医病机及治法。认为脱发的核心病机包括精血不足、气血失调,在标为风、热、湿、瘀,在本首责肾元不足、脾胃虚弱,提出燥湿相兼是脱发的主要病理状态,头皮毛窍湿遏于外则见油脂渗出,燥胜于内则见发质焦枯细软。归纳滋补肝肾、益气养血、祛瘀生新、润燥化湿、健脾和胃治法及主方、主药,并结合现代医学发病机制、治疗方法对其中医治法内涵进行探讨。文章阐述了脱发之治,临床应具备“治未病”理念,以斑秃为例,未病先防应防斑秃后白斑,既病防传应防斑秃向全秃、普秃发展,瘥后防复应防斑秃之复发;应注重雄激素性秃发之“治病求本”,防止脱发应注重风热之本,减少皮脂分泌应注重痰湿之本,促进毛发增长应注重脾肾之本。
Based on the overall concept and the classic theory of traditional Chinese medicine(TCM),the patho-genesis and treatment of hair loss are discussed from different dimensions such as viscera,qi and blood,nutrient and defen-sive qi,yang and yin fire,rise and fall,dryness and dampness,phlegm and blood stasis,positive deficiency,etc.It is be-lieved that the core pathogenesis of hair loss includes insufficient essence and blood and qi and blood disorders,which are labeled as wind,heat,dampness,and blood stasis.In this first place,it is responsible for the deficiency of the kidney,as well as the weak spleen and stomach.It is proposed that dryness and dampness are the main pathological state of hair loss.If the scalp is wet,the oil seepage can be seen;if there is internal dryness,the hair is dry and soft.This article summarizes the main prescription and main medicine of nourishing the liver and kidney,nourishing qi and nourishing blood,removing blood stasis,moistening dryness and dampness,and strengthening the spleen and stomach and discusses the connotation of TCM treatment in combination with the pathogenesis and treatment methods of modern medicine.The article elaborates on the treatment of hair loss.Clinically,it should have the concept of“curing the disease”with alopecia areata as an example,the alopecia and then leukoplakia should be prevented before the disease.The disease should be prevented from spreading to the development of total baldness and general alopecia,and its recurrence should be prevented.It is also necessary to pay attention to the root cause of androgenetic alopecia and the wind-heat to avoid hair loss;the foundation of phlegm and dampness should be highlighted to reduce sebum secretion,and hair growth can be promoted by focusing on the foundation of the spleen and kidney.
