摘要
目的:探讨解毒散外敷联合加味四妙散内服治疗湿热蕴结型急性痛风性关节炎的临床效果。方法:选取2021年8月—2022年7月阳春市人民医院收治的80例湿热蕴结型急性痛风性关节炎患者作为研究对象,根据随机数字表法分为对照组和观察组,各40例。对照组给予常规西药治疗,观察组给予解毒散外敷联合加味四妙散内服治疗。观察两组视觉模拟评分(VAS评分)、关节症状与体征评分,尿酸(UA)、肌酐(Scr)、内生肌酐清除率(Ccr)、临床疗效、不良反应发生情况。结果:治疗后,观察组VAS评分、关节症状与体征评分均低于对照组,差异有统计学意义(P<0.001)。治疗后,观察组UA、Scr水平均低于对照组,Ccr水平高于对照组,差异有统计学意义(P<0.001)。观察组治疗总有效率高于对照组,差异有统计学意义(P=0.048)。观察组不良反应发生率低于对照组,差异有统计学意义(P=0.013)。结论:解毒散外敷联合加味四妙散内服治疗湿热蕴结型急性痛风性关节炎的临床效果显著,可有效缓解患者疼痛并控制病情,且不良反应发生率较低。
Objective:To explore the clinical effect of external application of Jiedu powder combined with oral administration of modified Simiao powder in the treatment of acute gouty arthritis of damp-heat accumulation type.Methods:From August 2021 to July 2022,eighty patients with acute gouty arthritis of damp-heat accumulation type admitted to Yangchun People's Hospital were selected as the research objects.According to the random number table method,they were divided into control group and observation group,with 40 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with external application of Jiedu powder combined with oral administration of modified Simiao powder.The visual analogue scale(VAS score),joint symptom and sign score,uric acid(UA),creatinine(Scr),endogenous creatinine clearance rate(Ccr),clinical efficacy and adverse reactions were observed in the two groups.Results:After treatment,the VAS score,joint symptoms and signs scores of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.001).After treatment,the levels of UA and Scr in the observation group were lower than those in the control group,and the level of Ccr was higher than that in the control group,the difference was statistically significant(P<0.001).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P=0.048).The incidence of adverse reactions in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.013).Conclusion:External application of Jiedu powder combined with oral administration of modified Simiao powder has a significant clinical effect in the treatment of damp-heat accumulation type of acute gouty arthritis,which can effectively relieve pain and control the disease,and the incidence of adverse reactions is low.
