摘要
目的探究不同无创机械通气方式对呼吸衰竭患者下呼吸道感染的影响。方法便利选取2021年3月—2022年4月临海市第二人民医院收治的94例呼吸衰竭患者为研究对象,依据随机数表法将其分为对照组和观察组,各47例。两组患者均在治疗中运用无创机械通气,对照组采取单水平正压通气(CPAP)模式,观察组采取双水平正压通气(S/T)模式。对比两种不同无创机械通气方式对患者下呼吸道菌群的影响。结果治疗后,观察组VC、TV、MAP及RAW均优于对照组,差异有统计学意义(P<0.05)。治疗后,观察组PaO_(2)、PaCO_(2)及SaO_(2)均优于对照组,差异有统计学意义(P<0.05)。观察组下呼吸道菌群分布的病原菌群及下呼吸道感染率优于对照组,差异有统计学意义(P<0.05)。结论不同无创机械通气方式对呼吸衰竭患者下呼吸道菌群的影响作用明显,双水平正压通气模式更有助于肺功能改善,提高患者的机体免疫力,降低其防御机制受损的风险,减少口咽部定植菌的吸入,能够有效降低下呼吸道感染概率。
Objective To explore the influence of different noninvasive mechanical ventilation methods on lower respiratory tract infection in patients with respiratory failure.Methods A total of 94 patients with respiratory failure admitted to the Second People's Hospital of Linhai City from March 2021 to April 2022 were convenient selected as research objects,and were divided into control group and observation group according to random number table method,with 47 cases in each group.Both groups were treated with non-invasive mechanical ventilation,the control group was treated with single level positive pressure ventilation(CPAP)mode,and the observation group was treated with double level positive pressure ventilation(S/T)mode.The effects of two different noninvasive mechanical ventilation methods on the microflora of the lower respiratory tract were compared.Results After treatment,VC,TV,MAP and RAW in observation group were better than those in control group,and the difference was statistically significant(P<0.05).After treatment,PaO_(2),PaCO_(2) and SaO_(2) in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).The pathogenic flora and infection rate of lower respiratory tract flora in observation group were better than those in control group,and the difference was statistically significant(P<0.05).Conclusion Different non-invasive mechanical ventilation modes have significant effects on lower respiratory tract microflora in patients with respiratory failure.Bi-level positive pressure ventilation mode is more conducive to improving lung function,improving the body immunity of patients,reducing the risk of defense mechanism damage,and reducing the inhalation of oropharyngeal colonizing bacteria,which can effectively reduce the probability of lower respiratory tract infection.
